For one reason, this outfall collects water that runs off of adjacent property that is not under our control. Another reason is that the natural grade allows a majority of the water to flow towards outfall #1. 2. Attached is a page from our Storm Water plan that indicates AMEC, formerly MACTEC, pulls all samples that are required for our Storm Water sampling plan from outfall #2. 3. Attached is a site plan provided from AMEC, formerly MACTEC, which identifies the outfalls for our property. Although it doesn't show how the outfalls are connected, this is the only documentation I have for this. I've identified a drain that collects rain water from an adjacent property that comes out of outfall #2. Outfall #2 is our representative outfall but it does have storm water from other facilities. 4. We will have a short term fix for the diesel fuel containment unit with some patchwork. However, our plan is to remove the tank and place a liner in the containment to provide a watertight seal. We will provide pictures of the longterm solution once it's in place. The SDO inspection will be documented on the Stormwater Discharge Outfall (SDO) Qualitative Monitoring Report form provided by the NCDENR and maintained in the SPPP in Appendix B. 5.2.3 Semi-Anuual Quantitative Sampling Requirements The NPDES General Permit No. NCG030000 (Part 11, Section B) requires that analytical monitoring (i.e., sampling) be performed semi-annually. Semi-annual sampling will be performed at the facility once between January and June and once between July and December. A minimum of 60 days must separate sample dates. Samples need to be collected from a discharge resulting from a representative storm event, during the first 30 minutes of discharge from the outfall or "first flush" of the event. A representative storm event is a storm event that measures greater than 0-1 inches of rainfall and that is preceded by at least 72 hours in which no storm event measuring greater than 0.1 inches has occurred. Specific analytical requirements as listed in the permit are: total lead, oil and grease, total suspended solids (TSS), total toxic organics (TTO), total rainfall, and pH. It should be noted that the holding time required by the NCDENR for pH testing is 15 minutes. Therefore, pH measurements should be conducted on -site as opposed to being conducted at a laboratory. Of the four outfalls, SDO #2 receives the majority of the storm water runoff and based on the drainage area, has the most potential for contamination. Due to the similar nature of activities within the three outfalls, only SDO #2 is to be sampled during the general permit term. The inspection conducted found the following deficiencies of our Stormwater Pollution Prevention Plan (SPPP). The Facility Inspection report was not developed or implemented. Annual updates had not been made to account for the absence or presence of spills. The annual employee training records were not kept with the SPPP. The analytical sampling for the benchmark levels of copper as required in our permit. The lab needed to be contacted to advise if these detection limits could be met. The following list of items will be addressed in the following paragraphs. The Facility Inspection not being implemented or maintained was an oversight by Berenfield Containers and has been implemented. The stormwater discharge and non- stormwater discharge inspections were being completed a on a semiannual basis. The facility inspection form has been created and added to the SPPP. The inspection was completed on September 18, 2014 and the completed form has been attached to this letter for your review. The form was created to account for all areas of the property. The form is currently generic and we will review and update as more information becomes available on future inspections. If the Land Quality Section has a facility inspection form that is recommended for items listed or not listed that need to be added from your experience of reviewing other facilities inspection forms, please forward this form or item to Berenfield Containers. The annual updates on the presence or absence of spills on the property was not completed. Every employee at Berenfield Containers (BCSE) knows from the RCRA training received to watch for and report any occurrence of leaks, spills, and other problems should it occur. I do walk the property on a regular basis to insure that it is kept clean and to correct any possible problems that may occur. The annual training for the handling of hazardous waste and spill prevention and cleanup was conducted on September 10, 2014. The training record and program were kept in the Process Engineer's office as part the RCRA — Solid Waste program, Solid Waste Division of NCDENR. Berenfield will now keep a set of training records with the SPPP notebook for review by the inspector. The training includes the handling of hazardous waste and procedures for handling all types of spills inside and outside the plant. Berenfield Containers also maintains a Spill Locker with all the necessary tools and equipment to stop, contain, and clean up any spill that may occur. A copy of the Emergency Contingency plan that covers all the foreseeable events that could occur at the facility has been include within the SPPP notebook for future reference. The contingency plan addresses any spills that could affect the plant, property, or surrounding community are addressed with the Spill Team during our annual training. The analytical sampling and the testing procedure being used did not detect the benchmark levels as required in the permit. The requested limit of the permit is 0.007 mg/L for copper. The lab was testing the stormwater samples to a previously established test method and this method was unable to detect levels below a certain limit. I was unfamiliar with lab test methods and thought this was the best capability of the detection limit. Prism Labs was contacted and responded with the ability to change the test method from "200.7" to test method "200.8", which will be able to quantify the amount of copper detected to the required report limits and benchmarks set by NCDENR. The inspection conducted found the following deficiencies of our Stormwater Pollution Prevention Plan (SPPP). The Facility Inspection report was not developed or implemented. Annual updates had not been made to account for the absence or presence of spills. The annual employee training records were not kept with the SPPP. The analytical sampling for the benchmark levels of copper as required in our permit. The lab needed to be contacted to advise if these detection limits could be met. The following list of items will be addressed in the following paragraphs. The facility Inspection not being implemented or maintained was an oversight by Berenfield Containers and has been implemented. The stormwater discharge and non- stormwater discharge inspections were being completed a on a semiannual basis. The facility inspection form has been created and added to the SPPP. The inspection was completed on September 18, 2014 and the completed form has been attached to this letter for your review. The form was created to account for all areas of the property. The form is currently generic and we will review and update as more information becomes available on future inspections. If the Land Quality Section has a facility inspection form that is recommended for items listed or not listed that need to be added from your experience of reviewing other facilities inspection forms, please forward this form or item to Berenfield Containers. The annual updates on the presence or absence of spills on the property was not completed. The form for the annual inspections has been updated and will be maintained on an annual basis. Every employee at Berenfield Containers (BCSE) knows from the RCRA training received to watch for and report any occurrence of leaks, spills, and other problems should it occur. I do walk the property on a regular basis to insure that it is kept clean and to correct any possible problems that may occur. The annual training for the handling of hazardous waste and spill prevention and cleanup was conducted on September 10, 2014. The training record and program were kept in the Process Engineer's office as part the RCRA --- Solid Waste program, Solid Waste Division of NCDENR. Berenfield will now keep a set of training records with the SPPP notebook for review by the inspector. The training includes the handling of hazardous waste and procedures for handling all types of spills inside and outside the plant. Berenfield Containers also maintains a Spill Locker with all the necessary tools and equipment to stop, contain, and clean up any spill that may occur. A copy of the Emergency Contingency plan that covers all the foreseeable events that could occur at the facility has been include within the SPPP notebook for future reference. The contingency plan addresses any spills that could affect the plant, property, or surrounding community are addressed with the Spill Team during our annual training. The analytical sampling and the testing procedure being used did not detect the benchmark levels as required in the permit. The requested limit of the permit is 0.007 mg/L for copper. The lab was testing the stormwater samples to a previously established test method and this method was unable to detect levels below a certain limit. I was unfamiliar with lab test methods and thought this was the best capability of the detection limit. Prism Labs was contacted and responded with the ability to change the test method from "200.7" to test method "200.8", which will be able to quantify the amount of copper detected to the required report limits and benchmarks set by NCDENR. Berenfield Containers strives to be an environmentally conscious company and will do what is needed to meet the requirements of all permits assigned by NCDENR. A site visit was conducted on April 28, 2011, which found that stormwater discharged from the site to the unnamed tributary through a single stormwater outfall. The outfall consisted of the discharge from the stormwater detention pond on site. Due to vegetative growth the outfall from the pond was not inspected. During the site visit I discussed how qualitative and analytical monitoring should be conducted at the site to ensure compliance with the sampling requirements in NCG030000. Because the Oiles America stormwater pond provides stormwater detention, you are not required to conduct analytical or qualitative monitoring unless stormwater is discharging from the pond. If there is a discharge from the pond you should follow the monitoring requirements in Part II, Sections B and C and should collect a sample within the first 30 minutes of discharge. If there is no discharge from the Oiles America stormwater pond during any 6-month monitoring period noted in Table 2 of Part II, Section B then as noted in footnote #2 a Discharge Monitoring Report noting "No Flow" should be submitted to the Division within 30 days of the end of the six-month monitoring period. In reviewing the file, it was noted that a groundwater remediation system was in operation under an interim corrective action plan (ICAP) until 2002. At that time, Galvan received permission to de -activate the system but continue groundwater monitoring. In'2004, the DWQ requested that Galvan update the comprehensive site assessment (CSA) and amend the ICAP. This office received a report on February 12, 2006 that included an updated well survey but a work plan for the CSA was never received. A departmental reorganization in 2007 resulted in the transfer of the oversight of this incident to the Inactive Hazardous Sites Branch of the Division of Waste Management. It has been determined, however, that regulatory authority for this site should remain with the DWQ since the groundwater contamination occurred as a result of activities related to a DWQ permit (NC0005576). In order to meet the regulatory requirements, you must submit an updated site assessment report to the Aquifer Protection Section of the Mooresville Regional Office within 90 days of receipt of this Notice. This report must include an evaluation and discussion of the cited surface water impacts. The petition is for representative outfall status at your facility located at 7315 Millbrook Road, in Harrisburg, North Carolina. Ms. Corbin has petitioned the Division to allow stormwater discharge outfall 42 (SDO #2) to be representative of stormwater discharge outfall #3 (SDO #3). After discussion with Ms. Corbin and reviewing the submitted documentation, the Division staff concurs that SDO 42 and SDO #3 would contain the same types of potential pollutants. However, the Division staff does have a concern over the waste dumpster located next to the catch basin for SDO #3. Division staff has consulted with Ms. Corbin on this issue and she has indicated that the dumpster can be relocated. Therefore, the Division agrees with the petition and grants the request to sample SDO #2 as representative of SDO #3, under the condition that the aforementioned dumpster is relocated out of the SDO #3 drainage area. Please note that qualitative monitoring is still required at all outfalls at the facility regardless of representative outfall status as stated in Part II, Section C, Paragraph I of General Permit NCG030000. Florida Steel Drum Co., Inc. has received aNDPES permit, General permit No. NCO 0030000, Certificate of Coverage No. NCG 030253. The discharge was released from a collection and transfer station of our parts washer. The parts washer fiinctions to remove dirt and oil from the formed heads and bottoms of the 55 gallon drums we mamiiachure. The water contained washer type chemicals relating to soaps and neubralizers in low percentages. The normal operation of the collection tank is to collect the overflow from the parts washer and then pump this water to the sewer connector. This discharge is permitted under permit regulations with the Water and Sewer Authority of Cabarius County. Florida Steel Drum had recently performed the required analyses for sanitary sewer discharge and all areas were below the effluent limitations of the permit. They have been notified and are aware of the incident I was notified of the release at 9:00 am, 5/25/95. The Parts Department TeamLeader had already been notified of the release, had stopped the flow at the source, and had begun the clean-up procedure. The next concern was to survey the extent of the release. It has not rained recently End was not raining at this time. The storm drain contained some runoff water prior to the release. There are two large wash out areas in the storm drain which can hold about 500 gallons each before they fill and water can travel down the stream I contacted Rota -Rooter to ,dispatch a pumper tank to evacuate these two pit areas and return the water to the sanitary sewer, with the Water and Sewer Authority's permission. Roto- Rooter was able to complete this by 11:20 am, 5-25-95. Afler the spill was contained, I contacted your office and spoke to a Mr. Mike Parker. He took down my name, company, and location in case any calls were made from others in regard to this release. The parts washer transfer station has a submergible, pump used to transfer the water to the sanitary sewer.
