This site visit was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in NPDES WW Permit No. NC0020443. The findings and comments noted during this inspection are provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report". Page# NPDES yr/mo/day Inspection Type (Cont.) 31 NCO020443 I11 12 19/09/07 17 18 IOI Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) Robbie Bullock and Chris Pullinger visited the Columbia WWTP Post Hurricane Dorian to see if the facility had any issues from Hurricane Dorian. We noticed that the facility had a major sludge spill at the WWTP. We contacted the Tim Oliver (ORC) and had him meet us at the WWTP. Rhett White (Town Manager) and Tim Oliver met us at the WWTP. Tim stated that the facility lost power and the generator failed to start. This caused the Return Activated Sludge manhole to overflow for approximately 3 hours. Tim stated that he first saw the spill around 4 pm on Friday, September 6th and he did not think that it was a reportable spill. Tim was reminded that any spill at the WWTP is a bypass and is reportable. The spill was estimated at 198,000 gallons and a local septic pumper was hired to clean the spill up. Page# 2
