On Mon 6/19/17, our Environment, Health and Safety Compliance Manager (J. R. Concepcion) submitted a letter requesting
and Application of Representative Outfall. As of Mon 7124/17 we have not heard of a resolution from the NCDEQ — Division
of Water Quality. Back on Thursday 3/30/17 you have advised Life Safety Products, Monroe Operations ("Scott Safety") to
evaluate "representative outfall status" due to potential TSS exceedence caused by a nearby construction. Only stormwater
discharge outfall #3 or SDO-3 seems to be affected by the ConMet Aluminum Castings Plant Expansion.
On Friday 4/19/17, Mr. Concepcion notified "Scott Safety" Senior Leadership Team of a phone conversation in which you
proposed a site visit by Mrs. Holliday Keen, NCDEQ -- Union County. As of Mon 7/24/17, we have no records indicating that
1) Mrs. Keen has completed the site inspection or 2) that an exemption to sample SDO-3 was granted.
Just to make sure we can quantify the potential risk for NC030000 permit exceedence(s), we sampled the referred outfall on
Tuesday 5/23/17. "Scott Safety" received the lab results from Monark Industrial Services (att. Mark Johnson) on Thu 618117.
Copies have been provided in the Mon 6/19117 letter. As you can see in the chart below, SDO-3 readings were above
benchmark values and are at a historical peak:

IIt is worth to mention that the adjacent construction has reached a point in which we do not expect any changes in terrain
and/or landscape.
We have completed our first semi-annual Discharge Monitoring Report (DMR) on Thursday 3130117 and will be preparing for
our second semi-annual report in the next couple of months. Our concern is that "Scott Safety" has no ways to improve
stormwater quality at SDO-3 since, a maiority of the runoff is coming from the ConMet Aluminum Castings Plant Expansion.
We will appreciate any advice in this situation, prompt resolution is something we are driving with the highest priority. If you

In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COQ
• A copy of General Permit NCG030000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://Portal.ncdenr.orp/web/wq/ws/sulcurrent-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/i for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

Inspection Summary:
Scott Safety a Tyco International Company is located on approximately 35 acres of property at 4320 Goldmine Road in
Monroe North Carolina. The Scott Safety was inspected on August 17, 2017 for a technical assistance inspection reagarding
an offsite TSS impact for the stormwater. At the time of inspection, the facility was found to be in compliance with the metal
fabrication stormwater discharge COC. Although it was noted analytical sampling result on June 8, 2017 that SDO-3 reading
above the benchmark value due to construction next to the property. However it is recommended to sample a runoff from the
construction prior to getting to your outfall. It is recommended that the facility continue to perform the semi-annual sampling
for all the Outfall of the facility. SDO-4 was not accessible during the inspection. It is recommended to dear areas leading to
the Outfall for inspection and collection of sample purpose.

Hi Rob,
We've received an NOI from Norton Door Controls, a Division of Yale Securities,
Inc. in Monroe (Union County) for coverage of their door control hardware facility
under NCG03 (metal permit). The site discharges stormwater into a UT to Nelson
Branch. The NOI is attached.
They are a small generator of Haz Waste.
Does the Mooresville Regional Office have any concerns about issuing this facility
a COC for this general permit? If we don't receive any objections, we'll issue the
COC in 30 days.

The following are best management practices employed by Norton to control stormwater:
1) There are no oily waste materials stored outside.
2) Except for steel stored on racks, materials that must be stored outside on a
temporary basis are covered to prevent stormwater runoff.
3) There are administrative policies and procedures and physical structures in place to
prevent rainwater from certain areas such as, for example, the chip hopper area.
4) Containment areas are maintained to avoid to the maximum extent possible
stormwater runoff.
5) Contractors working on -site are trained prior to working on -site to avoid any
releases to any part of the site so that there is no stormwater runoff from their
activities.
