Parker with this Office. The enclosed report should be self-explanatory. Please note the comments located in the summary portion of the inspection report. It was noted by Mr. Parker during his inspection that a Stormwater and Pollution Prevention Plan (SPPP) has been developed for your facility, however, during his review of the Plan, several components of your Plan were lacking in detail as required by Part H, Section A of your permit. Furthermore, qualitative and analytical monitoring has not been performed as required by Part H, Section B. Such being the case, this inspection report is being issued as a Notice of Violation for the failure to comply with the terms and conditions of your General Permit. The SPPP did not include a list of significant spills during the past 3 years as required by Part ll, Section A, No. 1 (d). If there have been no significant spills in the past three years, it should be so noted in the SPPP. The BMP summary as required by Part II, Section A, No. 2 (c) did not provide in detail the existance of any BMPs or whether the use any BMPs are under consideration for pollutant removal. There was a brief mention of of a Spill Prevention and Response Plan (SPRP) in the SPPP, however, the personnel (or team) responsibile for implementing the SPRP was not identified. In addition, the SPRP should be stormwater specific as required by Part II, Section A, No. 3. Some employee training on spill response and clean-up is provided, however, it should be documented in the SPPP on an annual basis as required by Part II, Section A, No. 5. Based on information provided to the inspector during the inspection, there has been a recent change in the facility contact and this change has resulted in some difficulty in locating various records related to the SW permit and the SPPP, which was maintained by the former facility contact. These records included the possibility of any Qualitative or Analytical monitoring which may have been collected by the former facility contact.
