receiving analytical results.
No events occurred during the months of July through December, so no sample was collected from outfall 91.

The Mooresville Regional Office staff have reviewed your November 20, 2015 request to
remove outfall 2 from the stormwater sampling and analytical requirements as this outfall
captured rainfall from non -manufacturing area. Based on the information and pictures provided
with this request and a site inspection by the staff on January 5, 2016, we are approving this
request.
Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) or permit to
document that permission is granted for outfall2 to be removed from the sampling requirement.
If changes in drainage areas, structures, processes, storage practices, or other activities occur that
significantly alter the basis of this approval, permission to remove this outfall may no longer be
valid. You should either resume sampling at all SDOs, or reapply to this office for a new
permission based on updated information. If you have any questions or comments concerning

Mr. Khan,
The attached power point includes pictures that were taken during the dye test. Dye was poured on the edge of the
concrete pad where industrial materials are stored. The dye test was to evaluate if flow from the concrete pad would
cross the asphalt road and ultimately end up in the parking lot discharge (Outfall 002). All flow along the pad stayed on
the concrete and flowed to the drains in the pictures. These drains are tied to our storm water pond and ultimately to
Outfall 001. The pond has not had any discharges this year. The Plant Manager confirmed this for Samar via email.
Please let us know if you require additional information.

Dear Mr. Khan,
You may recall our Statesville, NC facility had requested permission to remove Outfall 2 from our storm water sampling
and analytical requirements as this outfall captured rainfall from non -manufacturing areas.
Mr. Bou-Ghazale visited the site earlier this year and after review indicated we could complete some dye testing and
verify storm water from the manufacturing storage area did not cross the road into the parking lot that this request
could be granted. He recommended completing dye testing during a couple rain events to verify flow direction.
The plant completed verification during a couple rain events and in late October sent the email below to Mr. Bou-
Ghazale. We anticipated a fairly quick response from him. As we have not heard from him I called the Regional Office
this morning to speak with him however was informed that he had left NCDENR for another job.
As he has departed, can you provide a response to our request to remove this outfall from the permit requirements?

Comment: The Stormwater Pollution Prevention Plan was last updated in July 2007.
There have been no spills within the last three years at the facility.
The permittee is training employees associated with the Stormwater Committee on an
annual basis. This training needs to be rolled out for "all personnel involved in any of
the facility's operations that have the potential to contaminate Stormwater runoff'.
Although the facility has reviewed and evaluated feasible alternatives to current
practices this review needs to be documented in the Stormwater Pollution Prevention
Plan.

Comment: No vehicle maintenance is conducted on site.
All concentrations of monitored parameters were below the cut-off concentrations in the
permit.
The permittee should review the requirement for TTO monitoring and move to
discontinue this monitoring since the Statesville facility does not use any solvents.

Comment: There are two designated stormwater outfalls at the Statesville facility. As
per the discussion during the inspection, the permittee does not need to sample the
sedimentation pond, to comply with the analytical monitoring requirements of the permit,
unless there is a discharge from the pond's riser -pipe.
There were liquid wastes pooling on the impervious surface below the trash compactor,
that could introduce contaminants to stormwater. discharges. However, the facility has
already identitifed this as a problem area and will be installing curbing to prevent mixing
of these waste liquids with' stormwater runoff.

Dear Mr. Stevens:
We are in receipt of your letter dated March 2, 2015 and received in our office on March 5,
2015 regarding the elimination of sampling requirement for the outfall 002 for the subject
facility. You have requested since Outfall 002 is located in the parking lot, thus it should
not be considered a discharge associated with industrial activities.
The Mooresville Regional Office staff inspected the subject facility on March 10,
2015 and met with Ms. Kristin Flora, Manager of Human Resources and Mr. Joe
Wegmiller, Manufacturing Director. During the site visit it was noted that discharge to the
parking lot directing towards outfall 002 may be coming from the metal storage area
located across the Asphalt Paved drive due to proximity of grade elevations and direction
of flow. It should also be noted that Zinc values at Outfall 002 sampling results shows
exceedance of the benchmark value.
In light of the above, it was agreed that The Goodyear Tire & Rubber Company —
Statesville Mold Plant is to provide evidence that the stormwater discharge from the metal
storage area is not discharging into the parking lot towards outfall 002. Please note that

We are not requesting representative status but rather that outfall 002 be exempted from the permit
requirements as it, in our opinion, does not meet the definition of a Storm water outfall Associated with
Industrial Activity as defined in the permit. Outfall 002 captures storm water from the office roof and the
employee parking lot only. We believe these are not Industrial Activities and that this outfall does not meet
the definition of an outfall associated with Industrial Activity as defined in the permit.
We request a site visit by the Regional Office to review the location of this outfall inspect the areas
discharging to this outfall and determine if this outfall is an Outfall Associated with Industrial Activity as
defined in the permit. -

The Goodyear Tire & Rubber Company — Statesville Plant (Goodyear) is covered under North Carolina General Storm
Water Permit NCG030000. Section B of the permit requires a notification should we exceed benchmark standards of
any pollutant four times during the life of the permit.
This notice is being submitted to inform the Division of Water Quality that we have exceeded the benchmark value for
Copper on 4 occasions from Outfall 2 for the above subject permit.
The benchmark value for Copper is 0.007 mg/L. Our recorded analytical values for this pollutant exceeded this value
as follows:
June 16, 2014 — 0.018 mg/L
December 5, 2014 — 0.009 mg/L
January 23, 2015 0.0114 mg/L
June 23, 2015 — 0.011 mg/L
We have conducted investigations in an attempt to identify the source of copper. Additionally we have cleaned the
storm water discharge lines from catch basins to the outfall and verified there is no flow during dry weather that could
be contributing contaminants.
We have reached out to the Regional office concerning the requirement for this outfall to be included in the sampling
requirements under the permit. Outfall 2 collects storm water from the employee parking lot, an office roof, and a grass
area in front of the office. There are no manufacturing materials stored such that they would contribute to this outfall.
A representative from the Regional office visited our site and we are presently conducting some tests to provide them
with information to verify this outfall does not contribute to the discharge from Outfall 2. Upon successful completion
of this work we believe the outfall could then be designated as non -manufacturing storm water. Sampling and
investigative work will continue until the issue is resolved.

We are in receipt of your letter dated March 2, 2015 and received in our office on March 5,
2015 regarding the elimination of sampling requirement for the outfall 002 for the subject
facility. You have requested since Outfall 002 is located in the parking lot, thus it should
not be considered a discharge associated with industrial activities.
The Mooresville Regional Office staff inspected the subject facility on March 10,
2015 and met with Ms. Kristin Flora, Manager of Human Resources and Mr. Joe
Wegmiiler, Manufacturing Director. During the site visit it was noted that discharge to the
parking lot directing towards outfall 002 may be coming from the metal storage area
located across the Asphalt Paved drive due to proximity of grade elevations and direction
of flow. It should also be noted that Zinc values at Outfall 002 sampling results shows
exceedance of the benchmark value.
In light of the above, it was agreed that The Goodyear Tire & Rubber Company —
Statesville Mold Plant is to provide evidence that the stormwater discharge from the metal
storage area is not discharging into the parking lot towards outfall 002. Please note that
once we receive the data, we will review it and respond to you accordingly as per our
discussion on site.

You may recall our Statesville, NC facility had requested permission to remove Outfall 2 from our storm water sampling
and analytical requirements as this outfall captured rainfall from non -manufacturing areas.
Mr. Bou-Ghazale visited the site earlier this year and after review indicated we could complete some dye testing and
verify storm water from the manufacturing storage area did not cross the road into the parking lot that this request
could be granted. He recommended completing dye testing during a couple rain events to verify flow direction.
The plant completed verification during a couple rain events and in late October sent the email below to Mr. Bou-
Ghazale. We anticipated a fairly quick response from him. As we have not heard from him I called the Regional Office
this morning to speak with him however was informed that he had left NCDENR for another job.
As he has departed, can you provide a response to our request to remove this outfall from the permit requirements?

We are in receipt of your letter dated March 2, 2015 and received in our office on March 5,
2015 regarding the elimination of sampling requirement for the outfall 002 for the subject
facility. You have requested since Outfall 002 is located in the parking lot, thus it should
not be considered a discharge associated with industrial activities.
The Mooresville Regional Office staff inspected the subject facility on March.10,
2015 and met with Ms. Kristin Flora, Manager of Human Resources and Mr. Joe
Wegmiller, Manufacturing Director. During the site visit it was noted that discharge to the
parking lot directing towards outfall 002 may be coming from the metal storage area
located across the Asphalt Paved drive due to proximity of grade elevations and direction
of flow. It should also be noted that Zinc values at Outfall 002 sampling results shows
exceedance of the benchmark value.
In light of the above, it was agreed that The Goodyear Tire & Rubber Company —
Statesville Mold Plant is to provide evidence that the stormwater discharge from the metal
storage area is not discharging into the parking lot towards outfall 002. Please note that
once we receive the data, we will review it and respond to you accordingly as per our
discussion on site.

In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG030000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part It of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.orglweb/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Units website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (0&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the fadlity receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

General Permit No. NCG030000 as attached.
This certificate of coverage shall become effective June 23, 2003
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
