Our records indicate that you have been issued the subject Certificate of Coverage (COC) under NPDES
General Permit (GP) No. NCG03000 from the North Carolina Division of Water Quality to discharge stormwater
associated with industrial activities performed at your facility. The purpose of this letter is to advise you that an
inspection is planned at your facility on November 8, 2011 at 11:00 am to ascertain compliance with the
aforementioned GP. If this date and/or time are not convenient for you or there is someone else with your company
that will be available to accompany me during my visit, you can contact me via email at
Michael.Parker@ncdenr.gov or at my phone number listed below to confirm your participation or to reschedule this
visit.
During the course of this inspection, you should be prepared to demonstrate your facility's compliance with
all terms and conditions as set forth in the GP. The following areas (if applicable) of the GP will be evaluated
during the inspection: Stormwater Pollution Prevention Plan, stormwater outfall locations, qualitative and/or
analytical monitoring data, and Representative Outfall status (if applicable).
If you are no longer the contact person for this facility, please provide this Office with the name of the
current contact person and a phone number where they can be reached so we can update our records. If you have

Your facility was recently granted coverage under stormwater general permit NCG030000. This letter serves as
your notification that the Division of Water Quality (DWQ) has reissued the general permit and that your facility's
coverage will continue under the conditions of the reissued general permit. Please review the new permit to
familiarize yourself with the changes in the reissued permit. The permit is reissued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina
and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A new Certificate of Coverage
A copy of General Stormwater Permit NCG030000
* A copy of the Analytical Monitoring Form (DMR)
* A copy of a Technical Bulletin for the general permit
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge stormwater from a facility located at
CONOVER PLANT
1506 EMMANUEL CHURCH ROAD
CONOVER
CATAWBA COUNTY
to receiving waters designated as a UT to McLin Creek, a class C stream, in the Catawba River Basin in accordance
with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III, IV, V, and VI
of General Permit No. NCG030000 as attached.
This certificate of coverage shall become effective September 1, 2002.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Mr. Gentry:
In accordance with your application for discharge permit received on January 25, 2002, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

This certificate of coverage shall become effective February 15, 2002.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
