However, our database does indicate that the permit rescission was approved by our Division on February 23, 2001. Please accept this letter as verification of the rescission, per your company's request, of your permit to discharge stormwater associated with industrial activity. If conditions change at your facility such that you are again discharging stormwater associated with industrial activity you will need to receive a NPDES permit for this discharge from our Division. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated December 6, 1983. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to request an individual permit by submitting an individual permit application. Unless such demand is made, this certificate of coverage shall be final and binding. Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of coverage. Metz, Safety / Environmental manager, Dana Spicer Clark - Hurth plant, Statesville. The hazardous materials generated by the facility are a by-product of their waste pretreatment facility. The hazardous materials consist primarily of oil and oil sludge. The material is contained in a holding tank until shipment. Materials are vacuumed directly from the holding tank to the transport vehicle. Safety - Kleen (Laidlaw Environmental) of Columbia, S.C. is the contractor for transport and disposal of the facilities hazardous waste. The facility averages 12 shipments per year.
