As noted by Mr. Parker during the inspection, there are specific sections of your Stormwater Pollution Prevention Plan (SPPP) that need to be addressed and/or updated. These areas.were discussed with you during the inspection and are also noted in the Summary Section in the attached inspection report. As a reminder, please note that your SPPP should be updated on an annual basis as required by Part I1, Section A, No. 7 of your permit. The general location map should include whether or not the receiving waters are impaired as required by Part Il, SectionA, No. 1(a) of the Stormwater Pollution Prevention Plan (SPPP). The SPPP should include a narrative description of the potential pollutants which could be expected to be present in the stormwater as required by Part ll, Section A, No. 1(b) of the SPPP, The SPPP should note on an annual basis whether or not there have been any significant spills during the previous year as required by Part Il, Section A, No 1(d) of the SPPP. The Spill Prevention and Response Plan should be included in the SPPP and the team members listed as required by Part Il, Section A, No. 3 of the SPPP. Employee training should be performed annually on proper spill response and cleanup procedures and the personnel (or team) responsible for implementing the training should be indentified in the SPPP as required by Part Il, Section A, No, 5. The SPPP should be updated annually as required by Part li, Section A, No. 7. The Certificate of Coverage available for review during the inspection had expired. The permittee has continued to be billed the annual compliance fee (and the fee has been paid), but no COC renewal has been received. This Office will inquire with the Central Office in Raleigh as to the status of this COC. During the site visit, it was noted that neither of the outfalls currently being sampled by this facility drain areas of the site where significant industrial activities take place. One outfall is in a grassed area and does not appear to receive drainage from the industrial process area and the other outfall is at the end of a concreted truck loading and unloading area located on the other side of the building from where the industrial activities occur. In addition, observations made by the writer during the site visit noted the possibility of this facility obtaining a No -exposure certification simply by eliminating some exposure sources at the rear of the building. COC Number NCG030411 Iredeil County Your facility is currently covered for stormwater discharge under General Permit NCG030000. This permit expires on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have the permit reissued by late summer of 2002. Once the permit is reissued, your facility would be eligible for continued coverage under the reissued permit. In order to assure your continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued coverage under the general permit. Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result in assessments of civil penalties of up to $10,000 per day. Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial facilities in any of the 1 I categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion.
