Operations Excellence, Inc. (OEI) has recently been engaged by Commercial Pabricators, Inc. to assist them with their stormwater issues. OEI plans to visit the site within one week of the date of this letter. We will perform a site reconnaissance, perform visual qualitative monitoring of the outfalls. and prepare / update the Stormwater Pollution Prevention Plan for the subject facility within two to three weep after our site visit. The plan will include information on monitoring and sampling and will also include an updated site map showing all outfalls. Once the plan is complete and reviewed by the client, we will perform a sampling event and submit the results to your office. Per our telephone conversation on November 26, 2007, we hereby request an extension until December 31, 2007 to implement the stomwater program at the facility. Facility representatives believe that a consultant may have prepared one for them, but they cannot locate it. They will contact the consultant to determine if there is a copy. As of 10126107, no Stormwater Pollution Prevention Plan has been provided. The facility did have a site map identifying some outside storage areas and two outfalls. An additional outfall was identified during the facility inspection. Additonal storage areas (solvent and hazardous waste), an additional outfall and facility changes (addition of more parking) must be added to the map and included in a storm water pollution prevention plan. A Storm Water Pollution Prevention Plan (SWPPP) must be developed for the facility and implemented 2. Solvent storage was located outside of the main building. There are two (2) secondary containment units containing MEK and mineral spirits. During the inspection, the valve on the large containment unit was open, which would allow accumulated solvents to flow out onto the ground. The small plastic containment unit was missing the valve cap which would also allow solvents to flow out onto the ground. All valves must be capped and preferably locked to prevent discharges of solvents to the ground. 3. A third outfall must be added to the site plan. Qualitative monitoring should be conducted on this outfall two (2) times per year. Qualitative and analytical sampling must be conducted on the other two (2) outfalls in compliance with the permitting requirements. 4. The facility must conduct and document routine facility inspections in compliance with the permit (at least twice per year). Corrective actions should be identified and implemented. 5. Upon permit renewal, analytical sampling must be conducted in compliance with the permit requirements. Analytical results must be submitted as required. Page: 2 Permit: NCG030394 Owner - Facifity: Commerical Fabricators Inc Inspection Date: 09/24/2007 Inspection Type: Compliance Evaluation Reason for Visit: Routine Comment: The pH at outfall No. 2 is out of range (result of 3.48). pH sampling not conducted on site, but sent to a laboratory. One sampling event was conducted in March 2006. It appears additional sampling was conducted in 2002 (an invoice for sampling was in the file from a consultant), but the results were not available on site. Facility representatives indicated that they do not store or use any products that contain a low pH. The facility must determine the source of the low pH and correct the issue. The facility did not have any records to demonstrate submittal of analytical monitoring data to the Division of Water Quality as required by the permit. Based on discussions with facility representatives, Total Toxic Organics sampling is not required. NCG030000 CERTIFICATE OF COVERAGE No. NCG030394 STORMWATER DISCHARGES NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the Federal Water Pollution Control Act, as amended, Commercial Fabricators, Inc. is hereby authorized to discharge stormwater from a facility located at 2045 Industrial Drive Newton Catawba County to receiving waters designated as Cline Creek in the Catawba River Basin in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, 11I and IV of General Permit No. NCG030000 as attached.
