On June 28, 2013, the Division of Energy, Mineral and Land Resources received your request to rescind your
coverage under Certificate of Coverage Number NCGO30276 In accordance with your request, Certificate of
Coverage Number NCG030276 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to waters
of the State without valid coverage under an NPDES permit is against federal and state laws and could result
in fines. If something changes and your facility would again require stormwater or wastewater discharge
permit coverage, you should notify this office immediately. We will be happy to assist you in assuring the
proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform the
new or prospective owners of their potential need for NPDES permit coverage.

Enclosed is a copy of the Follow-up Compliance Evaluation Inspection (CEI) report
regarding the compliance documentation submitted to Ms. Erin Hall of Charlotte -Mecklenburg
Storm Water Services from the subject facility on September 16, 2008. A follow-up site
inspection of the subject facility was not needed for the follow-up evaluation. This follow-up
process was conducted as part of a cooperative working agreement between Mecklenburg County
and the Division of Water Quality. In addition, the follow-up process was conducted on behalf
of the City of Charlotte in compliance with the City's NPDES Permit, NCS000240, Part II
Section H. Thank you for your assistance and cooperation during the follow-up compliance
evaluation.
The follow-up compliance evaluation found that Mount Hope Machinery has
implemented the corrective actions as stated in the permittee's response letter dated July 16,
2008. The corrective actions include:
• Updating the Stormwater Pollution Prevention Plan to include a USGS map with
accurate latitude and longitude coordinates for the points of discharge and
identifying Irwin Creek as listed on the NC 303(d) list and within a TMDL
watershed. The site map has been updated to include the percentage (%) of each
drainage area that is impervious and a narrative description of the potential
pollutants. The SPPP also contains a feasibility study and a site -specific Spill
Prevention and Response Plan.
• Evaluation of the stormwater outfalls for the presence of non-stormwater
discharges
• Completion of qualitative and analytical monitoring at all outfalls on site
• Correction of the issues identified during the initial inspection for the outdoor
IV storage area and the cardboard dumpster.

Inspection Summary:
Facility Description:
The Mount Hope Machinery Charlotte Plant specializes in the fabrication spreader rollers (new and repaired). Repaired
spreader rolls are assembled and finished then returned to the customer. The facility is located on approximately three
acres with one building. The building is surrounded by asphalt parking areas to the southwest and northwest. A wooded
buffer area is located to the north and east sides of the building. The facility's property line lies to the immediate south and
southeast of the building. The facility does not have aboveground storage tanks on site and no vehicle maintenance or
washing is conducted on site.
Compliance History:
Ms. Erin Hall of the CMSWS inspected the Mount Hope Machinery facility on May 20, 2008. The Division of Water Quality
issued a Notice of Violation on June 23, 2008.
Industrial Operations/City of Charlotte Stormwater Ordinance Review:
Outdoor Storage area -
During the initial inspection a 55-gallon drum containing hydraulic area was within the secondary containment structure
but was unlabeled.
Photo documentation submitted by the permittee on September 2, 2008, showed that the drum was properly labeled.
Open -top dumpster -
During the initial inspection three open -top dumpsters were observed for scrap metal, general waste products, and
cardboard. The cardboard dumpster had moderate rusting, no lid, and no drain plugs.
Photo documentation submitted by the permittee on September 2, 2008, showed that the current cardboard dumpster had
a rain lid and a drain plug.

Comment: The initial inspection of the subject facility on May 20, 2008 revealed that
the facility had not conducted a feasibility study of its current practices. The facility had
not developed a site specific Spill Prevention and Response Plan_ The facility had not
conducted annual reviews of its Stormwater Pollution Prevention Plan. The last
Stormwater Facility Inspection was conducted in 1995. The Stormwater Pollution
Prevention Plan had not been fully implemented due to the deficient general location
and site map items, not having a site specific Spill Prevention and Response Plan, not
conducting Stormwater Facility Inspections on a semi-annual basis, and not updating
their Stormwater Pollution Prevention Plan on an annual basis.
A review of the SPPP on September 16, 2008, found that the above noted deficiencies
had been corrected.

Comment: The initial inspection on May 20, 2008, found monitoring was only being
conducted at Outfall 001 although three outfalls were identified on site. Also, the
permittee last evaluated the stormwater outfalls for illicit discharges in 1995.-
The subject facility has submitted a Representative Outfall Status application to the NC
Division of Water Quality for review. All three outfalls must be sampled for analytical
monitoring until documentation has been received from the NC Division of Water Quality
concerning the subject facility's request for Representative Outfall Status. An illicit
discharge evaluation has been conducted.

inspection conducted at the subject facility on May 20, 2008, by Ms. Erin Hall of Charlotte -
Mecklenburg Storm Water Services. This inspection was conducted as part of a cooperative
working agreement between Mecklenburg County and the Division of Water Quality. In
addition, the inspection was conducted on behalf of the City of Charlotte in compliance with the
City's NPDES Permit, NCS000240, Part II Section H. Thank you for your assistance and
cooperation during the inspection.
This report is being issued as a Notice of Violation (NOV) due to the permittee's failure
to comply with requirements stated in the facility's general stormwater permit. This is in
violation of the subject Stormwater Permit and North Carolina General Statute (G.S.) 143-215.1,
as detailed in the Stormwater Pollution Prevention Plan, Qualitative Monitoring, Analytical
Monitoring, and Permit and Outfalls sections of the enclosed report. Pursuant to G.S. 143-
215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be
assessed against any person who violates or fails to act in accordance with the terms, conditions,
or requirements of any permit issued pursuant to G.S. 143-215.1.

Inspection Summary:
Facility Description:
The Mount Hope Machinery Charlotte Plant specializes in spreader rolls. Spreader rolls are fabricated at the facility as

Compliance History:
The permitteelfacility does not have any prior compliance issues with either the NC Division of Water Quality or
Charlotte -Mecklenburg Storm Water Services.
Industrial OoerationslCity of Charlotte Stormwater Ordinance Review:
Main Building —
AII manufacturing processes are conducted within this building, which does not contain any floor drains. A parts
washer was observed on a containment pallet. Crystal Clean picks up the parts washer waste on a monthly basis.
Small quantities of chemicals were stored in two flammable storage cabinets. Two loading docks are located at the
building. The main loading dock is located to the north of the building. No staining was observed in this area. The other
less frequently used loading dock is located to the southwest of the building next to the office. Minimal staining was
observed in this area.
Outdoor Storage Area --
One 55-gallon drum of hydraulic oil and one 55-gallon drum of MEK, a cleaning solvent, were observed within concrete
secondary containment under cover. No more than 110-gallons of total product are stored in the secondary
containment area at a time. The drum containing MEK was properly labeled, but the drum containing hydraulic oil was
not labeled. Several empty wooden shipping crates for spreader rolls were observed in this area on pallets. Other
general equipment was observed on pallets in this area.
It is recommended the facility properly label all drums.
Open -top dumpsters —
Three open -top dumpsters were observed on site. These include one for scrap metal, one for general waste products,
and one for cardboard. The scrap metal and general waste products dumpsters are located to the northeast of the
building. The cardboard dumpster is located to the north of the building next to Donald Ross Road. It had moderate
rusting and did not have rain lids or a rain plug. All dumpsters are picked up by different outside companies on an as
needed basis. A hopper was observed in the outdoor storage area. Per facility personnel, the hopper normally stays
inside the building.
It is recommended the.facilify contact Republic Waste Services and request a new cardboard dumpster with rain lids
and a rain plug.
Final product storage --
Spreader rolls are stored outside to the north and northeast of the building within elevated wooden shipping crates.
They are stored in this area as they are completed and await pick up.

Per facility personnel, monitoring samples are only being collected from Outfall 001. The current site map has a
stormwater catch basin designated as Outfall 003 located at the curb along Donald Ross Road. Samples taken from
this outfall would not be representative of the facility's stormwater runoff due to mixing with other stormwater runoff
along Donald Ross Road. However, the facility needs to obtain approval from the Division of Water Quality that the
stormwater discharge from Outfall 001 is substantially identical to the stormwater discharge from Outfall 002 and
Outfall 003 before the analytical monitoring requirements can be reduced.
No Exposure Certification
Upon completion of the inspection it was conveyed to facility personnel by the inspector that Mount Hope Machinery
may be eligible for a No Exposure Certification. The permittee should review Part I, Section A for additional information
regarding No Exposure requirements.
The permittee must adhere to all the requirements of the stormwater permit until a No -Exposure Certificate is secured.

Enclosed we provide our response for letter dated June 23, 2008 resulting from the stormwater
inspection at this facility on May 20, 2008.
As recommended for the industrial City of Charlotte compliance items have labeled the drum of
hydraulic oil in the storage area to note the contents. As recommended we contacted Republic
Waste Services and have been provided the rain protection for the open top dumpster. Attached
to this letter is a photo copy of the protective top liner.
On June 12, 2008 we submitted to your agency a request to identify our Outfall SW001 as the
representative outfall for our stormwater monitoring. We also submitted a request for a no
exposure certification that would exempt our facility from the analytical monitoring
We are in the process of preparing a Stormwater Pollution Prevention Plan to replace the existing
plan that will incorporate the changes recommended for specific location of each outfall,
inclusion of specific discharge and affected waterway, and development of spill prevention and
solid waste control plans. The new plan will be completed and implemented by August 30,
2008. The stormwater pollution prevention plan includes requirements for qualitative
monitoring inspections, semi-annual monitoring and reporting, outline for annual review of the
plan and any new recommendations, appropriate forms and guidance.

Your facility is currently covered for stormwater discharge under General Permit NCG030000. This permit expires
on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer of 2002. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will he expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion, For additional

Permits Unit Web Site at http://h2o.enr.state.nc.uS/Su/stormwater.html
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.
