Dear Mr. Klimek:
Pu meant to the requirements in Section IH(E)(S) of Copeland Corporation's ("Copeland") Storm
Water General Permit No. NCGO30000 (tire "Permit"), Copeland hereby submits this report
conning a release at its facility in Shelby, North Carohm
At approximately 3:00 AIt'L on May 14, 2005, Copeland experienced a release of approximately
70 gallons of a coolant -water solution into a neighboring storniWater ditch located outside of
Copeland's property. The sohrtion contained. 0.6% coon of a chemical called `Multan
RS". Therefore, approximately 4.2 gallons of Muhau RS was released during this -event. Muhan
RS is composed of the following (an MSDS is attached hereto for reference):
• Rust Inhibitor (10-30%)
• Trietbanolamine (1-10%)
• Diethanolamine (1-10%)
• Hexylene glycol (1-10%)
• �-(Butylamimol (1-10%}
• Fatty acid derivative (1-10%)
• Triazine (l -10%)
• Surlhctaid blend (Less than 1%)
• Petroleum distillates (Less than 1%).
I notified the National Response Center and the North Carolina Spill Hotline of this release at
approximately 11;00 Alit on May 14_ We investigated and determined that the cause of the
release was a blocked valve that could not close and seal because it was clogged with debris. We

have cleaned out the valve and have recovered all of the coolant from the stormwater ditch using
a sump sucker_ We have taken steps to reduce, eliminate, and prevent reoccunwce of a similar
release in the future by forming a team to address the issue; this team is currently in the proem
of drafting a corrective action plan.
Pursiant to Section M(E)(2)_ a copy of this report is being submitted to the Division of Water
Quality's Central Files.

Emergency Overview:
This produci is irritating to the eyes, respiratory system and skin. This product contains a component that may
cause allergic skin reactions. Long4erm exposure can cause lever and kidney damage. Prolonged or repeated
contad with liquid or vapor may affect the nervous system Some effects may be delayed.
Potential Health Effects:
Inhalation and skin contact are expected to be the primary routes of occupational exposure to this material. Based
on its composition. it is antidpated to be severely irritating to corrosive to eyes and skin. Prolonged or repeated
contact may remove offs from the skin and may dry skin and cause irritation, redness and rash. Repeated or
prolonged contact may cause an allergic skin reaction. High vapor conceptions may be irritating to the eyes
and respiratory tract, and may result in central nervous system (CNS) effects such as headache, dimness,
nausea, drowsiness and, in severe exposures, loss of eonsanusness. Precautions should be taken to limit
aerosolization or misting of this material, as repeated exposure to high concentrations of mineral oll mist may
cause lipoid pneumonia. If swallowed, this material may cause severe injury of the mouth, throat and digestive
tract and CNS effects as noted above. Mild to severe lung injury may occur if this material is drawn into the lungs
(pspirated) during swallowing, or during vomiting after swallowing. Symptoms of injury may include increased
breathing and heart rate, m4 thing and recited signs of respiratory distress. Repeated or prolonged costa t may
result in absorption of the material and systemic effects, such as limner or kidney injury. Medical conditions which
may be aggravated by exposure to this material include lung, Over or kidney disease or diminished function of

Eye Contact:
In case of contact with the eyes, rinse immediately with plenty of water for 15 minutes, and seek immediate
medical attention.
Skin Contact:
For skin contact, flush with large amounts of water. If irritation persrsts, get medical attention.
Ingestion:
If the material is swalkwed, get immediate medical attention or advice — Do not induce vomiting_ Give one to two
glasses of water or milk_ Never give anything by mouth to a victim who is unconscious or Is having convulsions.

Clean -Up Procefres:
Collect spOW material with an inert absorbent such as sand or vem*vMe. Place in properly 1a mIW dosed
container. Dispose of collected material according to regulation.
Spill or Leak
Contain spill. Stop leak at source if this can be done safely. Ventilate area. Nonessential personnel should leave
the area until clearap is completed. Pump riquid into DDT -approved drums for disposal_ Absorb remaining liquid
onto inert absorbent and place in DOT approved drams for disposal. Wash area with water. Keep concentrate and
wash water from entering sewers or waterways.
