Division of Water Resources’ Central and Regional staff has reviewed the application package
received June 25, 2019. However, additional information is required before the review may be completed.
Please address the items below no later than the close of business on September 4, 2019.
• According to the Wilkes County Tax Office, the owner of fields NC-WK-ED-03 and -04
(PIN 3888-17-5045) are Edward and Janet C. Staley of 1849 Old US 60 Hwy, Wilkesboro,
NC 28697 rather than June Irene Call. There is no Land Owner’s agreement (LOA) on file
for Edward and Janet C. Staley. Please provide a notarized LOA signed by the Staleys or
else indicate you wish this dropped from the permit.
• According to the Wilkes County Tax Office, the owners of field NC-WK-YF-07 (PIN
4847-38-6618) are Colen E. Warren and Nikki C. Warren of 6081 Pea Ridge Pass,
Hamptonville, NC 27020 rather than Mathis Properties LLC. There is no LOA on file for
the Colen E. and Nikki C. Warren. Please provide a notarized LOA Signed by the Warrens
or indicate you wish this dropped from the permit.
• There is no Land Owners Agreement (LOA) for Don Sparks and Mary Sparks for fields
NC-WK-DS-01 to -10. Please provide an LOA or else drop these fields from the permit.
• There were no soil sample results included with the application package for field NC-WK-
DS-04. Please provide soil sample results for this field.
• For fields NC-WK-DF-01 to -04, NC-WK-DS-01 to -03, and NC-WK-DS-05 to -08, no
soil sample results for Sodium, Exchangeable Sodium Percentage and Manganese were
provided. Please provide results for these parameters from these fields.
• In field NC-WK-DF-02, the USGS Map shows a stream flowing through the application
area that is shown on the submitted map as a drainage feature. Please show this as a stream
with the requisite setback. Also please recalculate the area and submit a revised Land
Application Site Certification form.

• The House of Raeford Farms, Inc. in Mocksville has changed ownership to Brakebush
Brothers, Inc. Please provide a revision to continue with this as a source.

still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items
above, or failure to provide the additional information on or before the above requested date may result in
