Does the site have a Stormwater Pollution Prevention Plan (SWPPP) or similar document? Is it site -specific?
Yes. Site follows NC DOT procedures for erosion control and NCGO10000.
Does the site have approved erosion and sedimentation control plans onsite? Are they representative of current site conditions?
Yes, the erosion control plans are onsite. They are continually updated with minor changes and repairs.
What type of stormwater training do site employees receive? How often?
Site is inspected by NC DWR/DOT inspectors monthly. Ms. Jo Anne Steinhaus of DWR and Mr. Jerry Yarborough have been
instrumental in conveying information on new requirements of the NCGO10000 to the contractors.

Inspector is NC DOT certified, amongst other qualifications.
Did the MS4 inspector appear knowledgeable about MS4 and NCGO10000 requirements for construction sites?
Yes. Discussed concrete washout issues and the ongoing problem of trucks washing out in random locations. Discussed forms and
rain gauge documentation.
Did the MS4 inspector appear knowledgeable about erosion and sedimentation control/storm water pollution prevention BMPs?
Yes. The inspector exhibited knowledge of erosion control techniques. Many of the issues with this site were early during the
project prior to swales and stormwater control measures were installed. Those issues were dealt with promptly and expertly.
During inspection, a crew was removing a small amount of sediment from the highway side swale(401/404) by hand.
