Does the facility have a Stormwater Pollution Prevention Plan (SWPPP) or similar document? Is it facility -specific?
The facility has a SWPPP that is site specific.
What type of stormwater training do facility employees receive? How often?
Employees watch a training video yearly. Additional training for employees is provided during safety meetings monthly. Ms.
Stephens receives training from Central Office throughout the year and disseminates information throughout the division.

Ms. Stephens receives training from Central Office throughout the year and disseminates information throughout the division.
Did the MS4 inspector appear knowledgeable about Permit requirements for pollution prevention and good housekeeping?
Ms. Stephens has never been inspected by this office. She did have a solid knowledge of the permit requirements. The members of
the stormwater team are well informed and knowledgeable of the permit requirements and good housekeeping, as a credit to Ms.
Stephens leadership.
Did the MS4 inspector appear knowledgeable about stormwater pollution prevention and good housekeeping?
Yes, Ms Stephens knowledge was evident during the inspection and proofed by the excellent condition of the facility.

Does the MS4 inspector's process include the use of a checklist or other standardized form?
Yes, Ms. Stephens uses standard DOT forms submitted to Central Office.

Does the MS4 inspector's process include taking photos? No.
Does the MS4 inspectors process include reviewing the facility's SWPPP (or similar document)?
Yes, the SWPPP is reviewed annually.
Does the MS4 inspector's process include walking the entire facility and inspecting all points of discharge?
Yes, Ms. Stephens inspects the facility per permit requirements, including all points of discharge.
Did the MS4 inspector miss any obvious areas of concern? If so, explain:
There are 5 stormwater ponds that are inspected by a unit from Burgaw. The stormwater ponds are an intricate part of stormwater
control. Formal documentation of inspection and repair must be included within the SWPPP. The ponds are continually maintained
and are in good condition.
Does the M54 inspector's process include presenting the inspection findings to the facility contact?
Yes. To begin the inspection, the M54 inspector informed the facility inspector what was going to happen, then conducted the
inspection, finishing with a summary of what just happened. A copy of the report will be sent to the inspector for NCDOT records.
Inspection Results
Did the facility inspection result in any corrective actions to be implemented? If so, for what issue(s)?
A building was under construction, nearly complete. This falls outside the immediate jurisdiction of this office in terms of erosion
control. However, additional groundcover was needed for the stream banks, stormwater pond, and bare and idle areas. Sediment
fencing needed repair.
Greater attention to erosion control for the stockpiles onsite is needed, including documentation of measures.
If compliance corrective actions were identified, what timeline for correction/follow-up was provided?
No timeline was identified. Ms. Stephens is to incorporate stockpile maintenance into her routine. The construction site is nearing
completion (less than one month). The stormwater pond documentation is to be incorporated into routine.
