Subject: Preconstruction Conference Minutes

A preconstruction conference was held at the Ahoskie District Office Conference Room located
at 230 NC 42 West, Ahoskie, NC on Wednesday, August 28, 2019.
A list of the attendees is attached.
The meeting was opened with a discussion of Environmental Issues.
Environmental Issues:
Kyle Barnes reviewed the Army Corps of Engineers Permit. Informed contractor that permits
need to be distributed to all contractors on project. Reviewed conditions in permit 1-18. The
contractor should notify the Army Corps of Engineers of any endangered species or historical
properties that may be encountered.
Garcy Ward reviewed the 401 Water Quality Certification. Garcy reviewed revised conditions 1-
23. Informed contractor that all pipes 48" or greater were to be buried one foot and pipes less than
48" were to be buried 20%. The widening of stream channels should be avoided and to notify him
when new stream location is complete.

Andy Blankenship and Paul Williams discussed erosion control issues. Andy verified that Site 6
was a total take.
Andy discussed differences between permit plans and erosion control plans. Brooks advised that
the designer has been notified.
All equipment shall be kept maintained and care should be taken to avoid contamination of
stream waters from fluids, fuels, etc. No staging of equipment of any kind is permitted in
waters of the U.S. or protected riparian buffers.
The contractor will maintain NPDES inspection records and immediately report any
violations to the Engineer, who will notify DWR within 24 hrs. of becoming aware of any
violations.
The tentative start date is October 1, 2019. The completion date for this project is March 13,
2024.
Direct contact between curing concrete and stream water shall be avoided.
Highly visible fencing shall be placed at the outer wetland boundaries before clearing begins,
and silt fence should be installed at the toe of slopes.
Only coir fiber matting is allowed in riparian areas. The use of matting which contains a
nylon mesh grid is prohibited.
Right of Way Issues:
Michelle reviewed parcels that were in conflict and advised that parcel 37 on sheet six needed
an additional PUE.
Brooks Braswell discussed contract issues.
Contract Issues:
The date of availability for this project is August 26, 2019 and completion date is March
13, 2024. Brooks reviewed Intermediate Contract Times and Liquidated Damages.
Brooks requested pay out schedule and progress report. Brooks also requested a
Supplemental Agreement for a Field Office for NCDOT personnel. Discussed culvert and
pipe designs for large fill areas and the use of vertical wick drains. Brooks reviewed
guardrail maintenance and DBE goals.
Brooks discussed the Major and Specialty items in the contract and the base index price
for fuel.
Clearing on this project shall be performed to the limits established by Method "III" shown
on Standard Drawing No. 200.02 of the 2018 Roadway Standard Drawings.
Jenine discussed DBE goals and advised contractor of subcontractor 1st and Goal needing
approval. Jenine also reviewed required documents that need to be posted for EEO
compliance.
Utility Issues:
James Pulley discussed utility relocation and requested URA for project. Start date and
estimated completion date not available currently. All other utility company's present are
following behind Dominion Power four to six weeks with their relocation.
Michael Kesick with Arcadis requested estimated cubic yard quantity of material in CSX
Railroad Right of Way. Michael reviewed pre -construction conference submittals and
requested to be notified when construction surveying begins. He informed contractor of
Railroad flagging procedures and to give flagging contractor seven days' notice when
services were no longer needed.
This completes the minutes of the Preconstruction Conference for Contract C204210 held on
Wednesday, August 28, 2019 at the Ahoskie District Office Conference Room located at 230
NC 42 West, Ahoskie, NC. Any discrepancies with the contents of these minutes should be
submitted in writing to this office. Failure to do so will be an indication of agreement.
