On October 13, 2015, Kenneth Friday of Charlotte -Mecklenburg Storm Water Services
(CMSWS) conducted an inspection of the above referenced facility as a requirement of the City
of Charlotte's NPDES Permit, NCS000240, Part II Section H. Inspection authority is granted by
Charlotte City Code Chapter 18, Article III, Section 18-82. At the time of inspection, the facility
was observed to be in compliance with the City of Charlotte Storm Water Pollution Control
Ordinance.
The inspection was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environmental Quality (NCDEQ) —
Division of Energy, Mineral and Land Resources (DEMLR). NCDEQ — DEMLR has been copied
on this report and made aware of the following observations regarding the facility's NPDES
stormwater permit:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and implemented.
2) Qualitative Monitoring
Qualitative monitoring has been conducted and recorded.
3) Analytical Monitoring

On August 26, 2015, Ron Eubanks and Andrew. DeCristofaro of Charlotte -Mecklenburg
Storm Water Services (CMSWS) conducted an inspection of the above referenced facility as a
requirement of the City of Charlotte's NPDES Permit, NCS000240, Part II Section H.
Inspection authority is granted by Charlotte City Code Chapter 18, Article II1, Section 18-82. At
the time of inspection, the facility was observed to be in compliance with the City of Charlotte
Storm Water Pollution Control Ordinance; however, several NPDES Stormwater Permit
violations were noted and the following recommendations are provided:
1) Charlotte Storm Water Pollution Control Ordinance Review
• Vehicle/Ecluipment Areas - Some of the compressors are washed in a bermed area on the
property prior to being repaired inside the warehouse. The wash water is recycled after
being treated in a filtration system located inside the warehouse. Although the wash pad
is bermed, it is recommended that the area be covered to prevent stormwater exposure.
• Waste Storage/Disposal Areas — There is one (1) municipal waste dumpster, one (1) scrap
metal recycling dumpster, and one (1) cardboard and pallet recycling dumpster located on
the property. Each of dumpsters appeared to be in good condition; however, the
cardboard and pallet recycling dumpster is uncovered. It is recommended that the
dumpster be covered to prevent exposure to stormwater and minimize polluted
stormwater runoff.

On May 21, 2015, Kenneth L. Friday of Charlotte -Mecklenburg Storm Water Services
(CM.SWS) conducted an inspection of the above referenced facility as a requirement of the City
of Charlotte's NPDES Permit, NCS000240, Part 11 Section H. Inspection authority is granted by
Charlotte City Code Chapter 18, Article 111, Section 18-82. At the time of inspection, the facility
was observed to be in compliance with the City of Charlotte Storm Water Pollution Control
Ordinance.
The inspection was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environment and Natural Resources
(NCDENR) ­:.Division,of Energy, Mineral and Land Resources (DEMLR). NCDENR — DEMI;R
has been copied on this report and made aware of the following observations regarding the
facility's Nil DES stonnwater permit:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP.) has been developed, recorded, and

Per the facilities permit, the SPPP is to be reviewed and updated annually.

On March 30, 2015, Dave Ferguson of Charlotte -Mecklenburg Storm Water Services
(CMSWS) sampled stormwater runoff from the subject facility located at 3628 Northerly
Road, Charlotte, North Carolina. The monitoring was conducted under authority granted
by Charlotte City Code Chapter 18, Section 18-82.
The monitoring was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environment and Natural
Resources (NCDhNR) — Division of Energy, Mineral and Land Resources (DEMLR).
NCDENR — DEMLR has been copied on this report and made aware of monitoring
results regarding the facility's NPDLS permit.
At the time the stormwater sample was collected, approximately 0.26 inches of rain had
fallen. Rainfall data was reported by the United States Geologic Survey (USGS) real-
time station CRN-53 (Harding High School). CMSWS sampled stormwater runoff from
Outfall 001.
A summary table of results is attached to this report. Monitoring results were compared
to local and state stream standards (CMSWS Action Levels and North Carolina Water
Quality Standards). Since the stormwater sample was not collected from the stream, the
stream standards do not necessarily apply; however, these standards can be used to
indicate the potential of polluted runoff from the facility to impact the receiving stream.
The results indicate elevated concentrations of total phosphorus ("I'll), zinc, and biological
oxygen demand (BOD). CMSWS recommends that you consistently implement good
housekeeping, pollution prevention, and preventative maintenance measures in order to
minimize contamination in your site's stormwater runoff.

Street, Charlotte, North Carolina. The monitoring was conducted under authority granted

results regarding the facility's NPDES permit.
At the time the stormwater sample was collected, approximately 1.14 inches of rain had
fallen. Rainfall data, was reported by the United States Geologic Survey (USGS) real-
time station CRN-15 located at the Charlotte -Mecklenburg Government Center.
CMSWS sampled stormwater runoff from the stormwater outfall located in the
southeastern corner of the property below the outdoor tipping area.
A summary table of results is attached to this report. Monitoring results were compared
to local and state stream standards (CMSWS Action Levels and North Carolina Water
Quality Standards). Since the stormwater sample was not collected from the stream, the
stream standards do not necessarily apply; however, these standards can be used to
indicate the potential of polluted stormwater runoff from the facility to impact the
receiving stream.
The sampling results indicate elevated concentrations of "Total Kjcldahl Nitrogen (TKN),
total phosphorus, turbidity, Biochemical Oxygen Demand (BOD), copper, lead, and zinc.
The results also indicate an elevated concentration of total suspended solids (I -SS)
compared to the NPDES Permit NCG130000 benchmark value of 100 mg/L for TSS.

The City's Phase I Permit was recently modified by the State and based on these modifications we no longer
have to notify the Mooresville Regional Office (MRO) of illicit discharges that reach the waters of the
State. We are currently providing this notification via a standardized email to Marcia. Effective immediately
you can discontinue doing this. We are now only required to notify MRO of discharges that constitute an
imminent threat to health or the environment.

On May 8, 2015, Ron Eubanks of Charlotte -Mecklenburg Storm Water Services
(CMSWS) conducted an inspection of the above referenced facility as a requirement of the City
of Charlotte's NPDES Permit, NCS000240, Part It Section H. Inspection authority is granted by
Charlotte City Code Chapter 18, Article III, Section 18-82. At the time of inspection, the facility
was observed to be in compliance with the City of Charlotte Storm Water Pollution Control
Ordinance; however, the following recommendations are provided:
Stormwater System — Sediment and concrete residue were observed in some of the catch
basins on the property. Inlet protection and routine cleaning of the catch basins are
recommended.
Structural Stormwater BMPs — There is a structural stormwater retention pond best
management practice (BMP) located on the property. There was a significant amount of
sediment and concrete residue observed in the retention pond. It is recommended that the
pond he routinely checked and cleaned out to ensure it functions properly.
The inspection was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environment and Natural Resources

of Charlotte's NPDES Permit, NCS000240, Part II Section H. Inspection authority is granted by
Charlotte City Code Chapter 18, Article 111, Section 18-82. At the time of inspection, the facility
was observed to be in compliance with the City of Charlotte Storm Water Pollution Control
Ordinance.
The inspection was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environment and Natural Resources
(NCDENR) —Division of Energy, Mineral and Land Resources (DEML R). NCDF_NR—DI71NLR
has been copied on this report and made aware of -the following observations regarding the
facility's NPDES stormwater permit:
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and
implemented.. The facilities (SPPP) was not reviewed and updated during 2014. Per the
facilities permit, the SPPP is to be reviewed and updated annually.

The inspection was also conducted as part of a cooperative working agreement between
Mecklenburg County and the North Carolina Department of Environment and Natural Resources
