The checked boxes below show what conditions were noted at your facility (also refer to the attached inspection report): 10 Failure to analyze the effluent: The effluent that is discharged from your system must be analyzed once each year. See Part [(A) of your permit about his requirement. Please make arrangements for sampling to be carried out within the next 3 months and submit results to this office -within 3 weeks after the sampling has been done. ® Other: An Engineer's Certification must be submitted upon completing construction and prior to operating this permitted facility. Based on a file review it does not appear that a Certification has been submitted to the Division and is due immediately along with as -built 121an drawings for any si ificant deviations from the ennitted design.
