O. box 1887 Greenville, N.C. 27835-1887 Dear Mr. Powell: d0k ED FE F1 November 27, 1995 Pitt County DEM Project # 951227 FILE COPY You have our approval to place fill material in wetlands and waters for the purpose of repairing a washed-out NPDES discharge culvert near the intersection of US 13N/NC 11 and US 264, as you described in your application dated 16 November 1995. After reviewing your application, we have decided that this fill is covered by General Water Quality Certification Number 3026. This certification allows you to use Nationwide Permit Number 3 when it is issued by the Corps of Engineers. This approval is only valid for the purpose and design that you described in your application. If you change your project, you must notify us and you may be required to send us a new application. For this approval to be valid, you must follow the conditions listed in the attached certification. In addition, you should get any other federal, state or local permits before you go ahead with your project. If you do not accept any of'the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 276 1 1-7447. This certification and its conditions are final and binding unless you ask for a hearing. The existing discharge culvert is undersized and will be replacad with a larger culvert. The cul•-ert has "washed-out" on one side and is creating an erosion problem. DRfDGR.: PRCJECr LEW IS, MC, Generc -on cctor - Z I II i I I I I i i Til-I p l I, V 1.0 Function/Scope The scope of this project includes: 1.1 Remove existing 42 inch RCP and replace with 48 inch RCP to eliminate erosion against the creek embankment and reduce water velocity. 1.2 Re-adjust and install existing wood deck with safety rail. 1.3 Dredge approximately 30 LF X 2 ft deep of the canal on the North side of the 48 inch culvert to remove mounds of mud and sediment restricting water flow. 1.4 Remove existing RipRap and install new RipRap with fabric in dredged area. 1.5 Design and Install a salt treated bulk head on the north side of the new 48 inch RCP.(See attached Sketch) 1.6 Fill and re-grade access road by the new 48 inch culvert. Fill material must be soils classified SM or better and compacted in accordance to Glaxo-Wellcome Co. Specifications. HOWSCOPE-2-92
