On March 8, 2019 Cory Larsen from Wake County Department of Environmental Services,
on behalf of the Division of Water Resources, visited the single-family residence (SFR)
located at 8201 Louisburg Road in Wake Forest to evaluate compliance with the above
permit to discharge wastewater. The checked boxes below show what conditions were
noted at your facility (also refer to the attached inspection report):
❑ In compliance: You are reminded to regularly maintain the chlorine disinfection
and dechlorination systems, have the effluent sampled once a year, and have the
septic tank pumped out every 3 to 5 years. Your good record of operation and
meeting the permit requirements is highly commended.
® Other: The system has not been constructed and the certificate of coverage is
expired. Please m letand submit the enclosed NCGS5 renewal form as soon as
possible. Failure to do so may result in further compliance issues.
