The inspection included a review of the Application Fields, Spill Plan and O&M Records. A review of the material loading area at the Fourth Creek WWTP was also performed as part of the inspection. The MRO would like to thank Alex Fox of Synagro for his time during this inspection. Please email a copy of the Laboratory TCLP and PAN results once the data has been received for the residual material as part of the inspection. The fields were in good condition and the material appeared to be being evenly applied across the application area. The attached report should be self-explanatory. There are no limiting slopes present on the any of these fields scheduled for land application. Mention was made that haul trucks transporting the residual cake to the LA fields will need to employ their retractable covers to prevent material from leaving the trucks while in route to the LA fields. A rain gauge was not observed during the site visit. Please be sure that a rain gauge is installed to measure precipitation during the LA event. A copy of the spill plan and O&M plan were also viewed during the site visit. All application haul trucks will have a copy of the permit and the spill plan present in the vehicles during the LA event. Monitoring Wells are not required as part of this permit. Soil samDles were collected before LA event. There appears to be adeauate separation between the surface and the top of groundwater. The fields appeared to be in good condition. All animals have been removed from the fields where residuals are scheduled to be applied. MRO staff observed the fields being buffered. All surface water and drainaae features were adeauatelv buffered to avoid residual material from entering any onsite surface water features. PAN was not reviewed during the site visit. TCLP analytical data should be forwarded to the MRO upon receipt of the results. Page 4 of 4
