Information Processing Unit:
Soil Plus, LLC (Soil Plus) is submitting all monitoring and reporting requirements as
specified in the above referenced permit for the Pasquotank County Water Treatment
Facility on behalf of Atlantic OBX, Inc. Soil Plus compiled this report using data gathered
during the compliance year. This data includes (1) certification form, (2) residual sampling,
(3) field loading calculations, (4) standard soil fertility nutrient analysis.
The Pasquotank County Water Treatment Facility conducted application events on
permitted fields in Pasquotank County during 2014. A total of 18.86 Dry Tons or 288,000
gallons of permitted residual bio-solids were land applied to permitted fields.

1. Only residuals approved for this permit were applied to the permitted sites
2. Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6
3. Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies of laboratory
results are attached
1. Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached
5. All other monitoring was performed in accordance with the permit and reported during the year as required and three (3) copies of
-ertified laboratory results are attached
5.The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T
7. All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part 503 regulated
:acilities).
3. All monitoring and reporting requirements in 15A NCAC 02T .1111 were complied with (applicable to 40 CFR Part 503 regulated

12. No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
13. All buffer requirements as specified on the permit were maintained during each application of residuals.

This report contains both routine soil test information as well as heavy metals data in a section labeled as Heavy Metals under soil test lab results. Using Mehlich 3 as a soil test
extractant, background levels of these metals typically seen in NC soils when analyzed are as follows: arsenic (As)- 4.5 ppm, cadmium (Cd)- 0.1 ppm, chromium (Cr)- 0.2 ppm, lead
(Pb)- 4.2 ppm, nickel (Ni)- 0.8 ppm, & selenium (Se)- 0.2 ppm (FY2005-2007). Although the above metals here are not believed to pose a concern for plant growth, continue to monitor
these and note where elevated above background levels. Where soil test phosphorus (P) is very high (P-I > 100), crops will not respond to additional P applied. Tissue testing is advised
where soil pH is elevated such as SS115. For soybeans and small grains, there is potential for micronutrient deficiency at elevated pH, with most concern of Mn deficiency. If deficiency
