On August 29, 2019 1 visited the Roby Conely Road facility. It was later discovered that the facility currently operates under
two separate Certificate of Coverage (COC) numbers. The COG number that is the most accurate in NCG100244. Therefore,
the permittee is directed to submit a rescission request for the Certificate of coverage number NCG100238.
