1c. Comments: See attachment for impact locations. Impact amounts are determined by assuming a 60 -foot long
by 10 -foot wide area would be disturbed to stabilize the bank and a 10
-foot by 15 -foot area would be needed
to replace the PVC pipe with ductile iron. No stream crossing will be
necessary.

1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
Disturbance to the buffer will be minimally necessary to access the repair site. Once work is complete,

1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction
techniques. Silt fence and appropriate erosion control measures will be established prior to beginning work.
2. Buffer Mitigation
2a. Will the project result in an impact within a protected riparian buffer that
requires buffer mitigation?
❑ Yes ® No
2b. If yes, then identify the square feet of impact to each zone of the riparian buffer that requires mitigation and
calculate the amount of mitigation required in the table below.

City of Raleigh Public Utilities Sewer Maintenance staff discovered an area of streambank failure near
905 Applenook Court, Wake Forest (Lat. 35.9670, Long. -78.5455) along an unnamed tributary of
Richland Creek. Approximately 60 linear feet of streambank will be stabilized. Upstream of the proposed
streambank stabilization, approximately 3 linear feet of 8" PVC sanitary sewer pipe will be replaced with
restrained -joint ductile iron pipe. The repair area is located within a maintained sanitary sewer
easement.
The following activities are proposed at this location:
• Erosion control measures and a dry work area will be established prior to work beginning.
• The bank will be stabilized with rock/geotextile.
• 8" PVC pipe will be removed and replaced with 8" ductile iron pipe
• All disturbed areas will be seeded and mulched upon completion of the work.
