4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The general land -use in the vicinity of the project is characterized as commercial development. Retail and commercial structures are located west, south, and east of the intersection of
NC-53 (Western Boulevard) and Gateway Drive North. Undeveloped land best characterized as Mixed Deciduous -Coniferous Forest Land is located north of this intersection. A
recreational park (Richard Ray Park) is located approximately 500 feet east of the project.
An unnamed perennial stream crosses under Gateway Drive North near its intersection with NC-53. This stream currently flows through a 36-inch reinforced concrete pipe (RCP) for
106 linear feet under Gateway Drive North. A catch basin (CB) and drop inlet (DI) located on the northwestern and southeastern side of Gateway Drive North, respectively, transfer
water from the surface to the RCP. A metal and wood retaining wall has been constructed along a driveway access for commercial shops that is to the southeast of Gateway Drive North
This retaining wall is located along and to the south of the unnamed perennial stream.
There are other existing drainage pipes on site that convey water from ditches that have been deemed non -jurisdictional.

4h. Explain the purpose of the proposed project:*
The purpose of the proposed project is to implement intersection improvements for two (2) intersections: NC-53 (Western Boulevard) at Jacksonville Parkway/Gateway Drive North and
NC-53 (Western Boulevard) at Gateway Drive South. Improvements are required to accommodate additional traffic volume associated with the proposed Jacksonville Marketplace site,
and the anticipated future growth rate in the region.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The widening of the existing roadway to accommodate proposed turn lanes will require fill slopes in some areas to match existing elevation. These fill slopes will be constructed at a 3:1
elevation ratio when adjacent to streams/wetlands, with the exception of Site 2 (reference in the Wetland and Surface Water Impacts Permit Plan Sheets, attached), in which a 2:1
elevation ratio will be used. Because of the steeper slopes, a guardrail will be constructed in this portion of the project.
The existing 36-inch RCP will be removed and replaced with 132 linear feet of 60-inch Class IV RCP. The existing CB will be removed and replaced with a CB at the edge of the added
turn lane, and the existing DI will be removed and replaced at the same location. The Class N RCP will be buried 1 foot into the ground. At the northwestern outlet, Class I rip rap on top
of a GeoTextile fabric will be placed on the stream bank for stabilization. Rip rap will not be placed in the stream channel. At the southeastern outlet, a headwall will be constructed.
A temporary construction easement will be needed on the northwestern side of Gateway Drive North for the removal and installation of the RCP and construction of the fill slopes. A
permanent drainage easement will be needed on the southeastern side of Gateway Drive North to install the RCP and headwall and allow future maintenance of the RCP.
Standard road construction and pipe removal/installation equipment, such as excavators, cranes, and dump trucks will be used for the project.

E. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
For most of the project, fill slopes were designed at a 3:1 foot elevation ratio. For site 2, the slopes were steepened to a 2:1 foot elevation ratio, which
avoided impacts to the perennial stream near this site and reduced wetland impacts from 0.030 acres to 0.011 acres.
The newdrainage pipe vdll be buried approximately 1 foot belowthe natural stream bed, which will facilitate conveyance of aquatic organisms and help
prevent scour holes. The current drainage pipe shows evidence of a perched culvert, which is likely currently preventing the movements of most
aquatic organisms.
Riprap at the northwestern outlet of the drainage pipe will not be placed in the stream channel to avoid permanent impacts to the stream. The riprap
and GeoTextile fabric will also minimize sedimentation of the stream.
A headwall will be constructed to avoid permanent impacts to the portion of the unnamed perennial stream that is southeast of Gateway Drive North.
This headwall will also prevent impacts to the surrounding wetland via fill slopes.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
A silt fence will be placed along the limits of disturbance to discourage sedimentation of the stream. Appropriate sediment and erosion BMPs have
been developed for the project.
