Is it site -specific? Operated under NPDES construction requirements. Grading and stabilization plan could also be considered a SWPPP style document for this type of activity. Does the site have approved erosion and sedimentation control plans onsite? Are they representative of current site conditions? Yes. Plans were redlined to show current conditions and any changes that may have been made. What type of stormwater training do site employees receive? How often? Employees receive a general stormwater training once a year. . Inspector Training/Knowledge What type of stormwater training does the MS4 inspector receive? How often? MS4 inspector receives the same stormwater training as other site employees and receives it once a year. Did the MS4 inspector appear knowledgeable about MS4 and NCGO10000 requirements for construction sites? MS4 inspector appears knowledgeable about the sediment requirements. Some of the requirements of other areas of concern, such as filling stations, concrete washouts, and vehicle storage were not understood as well. Did the MS4 inspector appear knowledgeable about erosion and sedimentation control/stormwater pollution prevention BMPs? MS4 inspector appeared to be very knowledgeable about the E&SC BMPs. MS4 inspector understood the maintenance requirements and conditions that triggered maintenance. NCS000250_NCDOT MS4 Facility inspection_2019 Page 4 of 7 Site Visit Evaluation: Construction Site No. 1 Inspection Procedures Does the MS4 inspector's process include the use of a checklist? MS4 inspector did not have a checklist of the monthly checks. Was told he used the previous month's report as a checklist. A checklist was used during the weekly inspections. Does the MS4 inspector's process include taking photos? Photos were taken when issues were spotted or if something needed addressing. Does the MS4 inspector's process include reviewing approved plans (erosion and sedimentation control and/or SWPPP)? Yes. MS4 inspector looked at the redline plans that were on -site to ensure he inspected the current area of work. Does the MS4 inspector's process include walking the entire site and inspecting all points of discharge? MS4 inspector looked at BMP discharge points. Not all rock outlets in the silt fence were observed during the inspection. Did the MS4 inspector miss any obvious violations? If so, explain: MS4 inspector went right past the onsite fuel tanks without inspecting for potential damage or leaks. MS4 inspector did not know if his fuel tanks were double walled or not. Concrete washout pits were not lined, although they were far from surface water one was located directly up gradient of a drop inlet. Washouts should be lined as a secondary containment measure. Does the MS4 inspector's process include presenting the inspection findings to the site contact in writing? MS4 inspection report was given to all NCDOT personal on site and was made available to the contractors on site. Does the MS4 inspector's process include providing construction stormwater educational materials to the site contact? This was not observed during the inspection. Compliance/Enforcement What, if any, enforcement actions (verbal warnings, NOV, etc.) did the inspection result in? MS4 inspector was informed to start inspection all areas of concern, not just sediment basins. Site contacts were also told to line concrete washouts, they informed NCDEQ it was not in their contract to do so. If compliance issues were identified, what timeline for correction/follow-up was provided? N/A Notes/Comments/Recommendations Site was in decent condition. Site had a blowout that released sediment to a wetland. During the inspection NCDEQ was informed that it was reported to Division of Water Resources, however no communications were given to show this. Concrete washout pits SHOULD BE LINED. The goal of the washout is to protect not only surface water but groundwater as well and without a liner infiltration can occur. Onsite fuel tanks were in a hazardous area where they could be subject to collision with vehicles, they should be inspected frequently to ensure they were not damaged and leaking. Rip rap channel and rock outlets were the only BMPs on site but were well maintained and regularly inspected. Inspection Procedures Does the MS4 inspector's process include the use of a checklist? Yes. The weekly inspections and monthly inspections were done using the same checklist. Does the MS4 inspector's process include reviewing approved plans (erosion and sedimentation control and/or SWPPP)? Yes. MS4 inspector ensures that the current conditions are visible on the redline plans. Does the MS4 inspector's process include walking the entire site and inspecting all points of discharge? Yes. MS4 inspector looks at all areas where sediment could enter the creek and inspects the silt fence around the storage/parking area. Did the MS4 inspector miss any obvious violations? If so, explain: No. Does the MS4 inspector's process include presenting the inspection findings to the site contact in writing? Yes. A report is generated and left on site for the weekly and monthly inspections. Does the MS4 inspector's process include providing construction stormwater educational materials to the site contact? This was not observed during the inspection. Compliance/Enforcement What, if any, enforcement actions (verbal warnings, NOV, etc.) did the inspection result in? None. If compliance issues were identified, what timeline for correction/follow-up was provided? N/A Notes/Comments/Recommendations Site was in very good condition. Size of site allowed for more regular inspections. MS4 inspector informed NCDEQ that when a storm is in the forecast they will inspect prior to and after the storm to ensure no changes were made to the erosion and sediment control devices.
