4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The site is steep embanked stream abutted by Substation yards. the Eastern most is dormant and the Western is Active. The site has a small Utility bridge supporting conduit. After
hurricane Matthew, in late 2016 it was discover one of the aprons was completely undermined, along with a fence footing creating a security and public safety issue, due to an
embankment failure along the stream in the site at various sites along it's length.

The project is a least impact method to halt embankment erosion from the bridge at filter plant drive to the downstream utility bridge. allowing PWC to re -secure the substation from the
public, and eliminate future storm erosion concerns.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
After restoring the integrity of the Utility bridge apron.The project will be to armor each embankement. The earth remove will be used where possible to restore a fertile upper
embankment. this is achieved by using bank -top backhoes. the other earth moving equipment will be a bulldozer and a dump truck as needed. To remove existing soil and replace them
With NCDOT specification Gabion Baskets linked together from stream base elevation to 9' above. the backfill will then slope back @ 1:1 slope to meet existing grade. This will be
seeded with quick growing sediment erosion mix with straw matting and live stakings (modified to root ball stakings) for upper bank stabilization. During the project a low sandbag wall will
halt water flow to be diverted/ de -watered to the project end and discharged into the existing class 4 stone currently in place. This is prevent sediment migration during construction.

We felt we comply with Army Corps Requirement for NW13, but do not fall within its jurisdiction

Rather than step or lay back a 12-15' deep excavation -45'+ disturbance/if stream, we were able to reduce that to approximately 15'. By installing the
Gabion Baskets we reduce disturbance size and duration.

The gabion baskets mimic the original extreme slopes allowing us to restore the stream embankments to near original.

MEASURES AND ALL DE -WATERING
5. ALL BACKFILL SHALL BE MATERIAL WELL GRADED GRANULAR SOIL SUITABLE
FOR COMPACTION, UNLESS NOTED, AND PLACED IN 8" LIFTS MAXIMUM, AND
SHALL MEET 95% PROCTOR DENSITY COMPACTION AS PER ASTM D698. AS
SIGNIFICANT CLAYEY MATERIAL WILL NOT BE PERMITTED.
6. CONTRACTOR SHALL HANDLED AND REMOVE ALL REFUSE AND WASTE MATERIAL,
PER ALL LOCAL STATE AND FEDERAL LAWS.
7. GABION WALLS SHOULD FOLLOW NATURAL EMBANKMENT CONTOURS AS MUCH
AS IS PRACTICABLE.
8. A SECTION OF GABION WALL IS AN ADD ALTERNATE TO THE BID CONTRACT AN
MAY BE INCLUDED IN THE CONTRACT AT THE DISCRETION OF FPWC.
