Concrete batehing operations. A unique element is that there will be no planned vehicle maintenance or washdown performed at this
location. Truck washdown will be performed on the adjacent construction site that the portable plant will be servicing. 'Truck wash-
out that occurs at the end of the day (delivery day) will be performed where the tricks originate: typically the existing plant at 400 N.
Long St., Salisbury. No trucks will remain at the facility when idle. Aggregate watering will be the primary PW source.

• Two (2) site plans depicting the facility or site with numbered outfalls which indicate stormwater and
wastewater outfalls. The plans shall legibly show. at a minimum, (existing orproposed):

been setup adjacent to the plant will also utilize port -a -potties. Fresh water is provided from municipal
supply.

performs similar policy or decision making functions for the corporation, or (b) the manager of one or more manufacturing production or
operating facilities employing more than 250 persons or having gross annual sales or expenditures exceeding 25 million (in second quarter
1980 dollars), if authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
-r (1) For a partnership or sole proprletorslilp. by a general partner or fire proprietor, respectively, or
(2) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected
official.
Certification: North Carolina General Statute 143-215.6 B(1) provides that:

document filed or required to be maintained under this Article or a rule implementing this Article; or who knowingly makes a false statement of
