Your facility was sent a notification in March 2009 to renew your Animal Waste Operation State Non -Discharge General Permit that
expired on September 30, 2009, Our records show that you have not submitted a renewal application to this offide as of today.
ljryou are a facility that is out of business, has depopulated, and is waiting for the lagoons) to be closed, you may request an
individual Permit with zero animals Please indicate so on the attached form and you may be waived the annual fees and 01C
requirements by obtaining the individual permit until all the lagoons have been closed and ,your permit rescinded.
In order to assure your continued overa a under the State Non -Discharge General Permitsyou must submit an a lication for ormit
coverage to the Division. Enclosed you will find a `Request for Certificate of Coverage Facility Currently Covered by an Exgirins
State Non -Discharge General Permit.' The =ligation form must be completed and returned by December 15. 2009. Please note. -that
you, must include two (2) comes of your most recent Waste Utilization_ Plan with the application form.
Failure to request renewal of your coverage under a general permit within the time period specified may result in a civil penalty.
Operation of your facility without coverage under a valid general permit would constitute a violation ofNCGS 143-215.1 and could
result in assessments of civil penalties of up to $25.000 per day.
