Your facility was sent a notification in March 2009 to renew your Animal Waste Operation State Non -Discharge General Permit that
expired on September 30, 2009. Our records show that you have not submitted a renewal application to this office as of today.
If you are a facility that is out of business, has depopulated, and is waiting for the lagoons) to be closed, you may request an
individual permit with zero animals. Please indicate so on the attached form and you may be waived the annual fees and 0IC
requirements by obtaining the individual permit until all the lagoons have been closed and,yourpermit rescinded.
In order to assure vour continued coverane_under the State NQn Discharge General Permits, vou_must submit an annlication forpermit
coverage to the Division. Enclosed you. will find a `Request for Certificate of Coverage Facility Currently Covered by an ExRirine
State Non -Discharge General Permit.' The annlication form must be completed and returned by December 15, 2009. Please note_ that
you must include two (2) copies of your most recent Waste Utilization Plan with the annlication form.
Failure to request renewal of your coverage under a general permit within the time period specified may result in a civil penalty.
Operation of your facility without coverage under a valid general permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $25,000 per day.
