This is a reminder that you are registered with this Division as a Manure Hauler. Your Certificate of
Registration for Manure Hauler Operations contains a condition that addresses the annual report
submission requirement as stated below:
...each manure hauler operation must submit an annual report to the Division by March I of each
year. The report covers the activities of the previous calendar year and must contain all applicable
information required by 15A NCAC 02T .1404. "
Please let us know if you are still in business. If so, please use the enclosed blank forms to
complete your annual report for 2010 and any revious year for which the report is still due.
Feel free to make additional copies for your future use. To avoid any possible enforcement action
for a violation of your Certificate, please complete and submit the report by March 1.2011 to the
