This is a reminder that you are registered with this Division as a Manure Hauler. Your Certificate of
Registration for Manure Hauler Operations contains a condition that addresses the annual report
submission requirement as stated below:
"....each manure hauler operation must submit an annual report to the Division by March 1 of each
year. The report covers the activities of the previous calendar year and must contain all applicable
in required by 15A NCAC 02T. 1404.
"
Please note that your new Certificate Number is MHC000008. Let us know if you are still in
business. If so, use the enclosed forms Haul-1 and either Haul-2 or Haul-3 (depending upon the
size of your operation) for 2011 and anyprevious year for which the annual report is still due.
Feel free to make additional copies of these forms for your future use. To avoid any possible
enforcement action for a violation of your Certificate, please complete and submit the report by
March 1, 2012 to the following address:
