This is a reminder that you are registered with this Division as a Manure Hauler. Your Certificate of
Registration for Manure.Hauler Operations requires an annual report as stated below:
"....each manure hauler operation must submit an annual report to the Division 6y March 1 of each year.
The report covers the -activities of the previous calendar year and must contain all applicable information
required by 15A NCRC 02T .1404. "
Please let us know if you are still in business. If so, please use the enclosed forms Haul-1 (less than
750 dry tons) or Haul-2 (greater than 750 dry tons) to submit your annual report for 2013 and any
other years for which the annual report has not been submitted yet. Feel free to make additional
copies of these forms for your future use.
To avoid any possible enforcement action for a violation of your Certificate, please complete and
submit the annual report within 30 days of receipt of this letter to:
