Below is a summary of your comments and the revisions made to the drawings to address each comment. Original comments are in italicized text, and responses are in bold text. A. Site Map (15A NCAC 02T .0105): 1. The unnamed tributary appears to bean intermittent stream, and not a water diversion. The unnamed tributary was relabeled as an intermittent stream and the setback buffer increased to 100'. This can be seen on Sheet 01. 2. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites. Previously a buffer of 400 feet was shown. This was increased to 500 feet and a darker linetype used. This can be seen on Sheet 01. 3. Location and identification of each monitoring well (Using names according to Attachment C of the permit). The names of the monitoring wells was updated in the table shown on Sheet o1 to match what is listed in Attachment C of the existing permit. The names were also updated on the plan view as well on Sheet 01. 4. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits). The latitudes and longitudes originally shown in degrees, minutes, seconds, were converted to decimal degrees in the table on Sheet 01. 5. The wetted perimeter of all imgationrnfiltration fields with field names (named according to the approved permit). The wetted perimeter of all Fields can be found on Sheet 03. Additionally, a summary table was added to Sheet 03 that shows the field names, the wetted acreage, the wetted area radius, and the sprinkler heads for the 4 newer fields. 6. The delineation of compliance and review boundaries. The compliance boundary is either 250 feet from the wetted perimeter or 50 feet within the property boundary, whichever is closest to the irrigation area. The compliance boundary cannot cross onto property owned by separate entities. The compliance boundary was updated so that it was either 250 feet from the wetted perimeter or 50 feet inside the property line, whichever was closer, and did not cross onto an adjacent property owner's site. B. Major Modification f 15A NCAC 02T .05041: 1. Provide a process flow diagram of the proposed treatment system (including RO system). The RO system is shown, however, the RO system was installed strictly to meet the requirements of the Yadkin Valley Sewer Authority (YVSA) for discharging to their treatment system and does not discharge to the spray fields. The permit for the RO system is currently in the process of being obtained under a separate permit application. There is no physical interconnection between the Reverse Osmosis system and the 420,000 irrigation ponds that discharge to the spray fields. This can be seen on updated Sheet 04. Additionally PDF drawings directly related to the DAF and Beltpress have been added to the drawing set as Sheets 13 and 14 respectively. 2. Provide engineering plans and details of the reverse osmosis system. We are providing details of the RO system for information only as the RO filtrate goes directly to the lift station, a purified water holding tank that is used for reuse, or back to the clarifier only. This can be seen on the updated PDF shown on Sheet 04. Drawings for the RO system are included as Sheets 15 through 36. ENGINEERING CERTIFICATION Documentation of any variation to this permit, and the Division -approved plans and specifications, is in the attached as -built drawings. Description of variations: Observation was not done during construction but a site visit was done on Friday_, June 7 2019 during which the conditions shown in the attached record drawings were compared to the existing conditions in the field and observed to be accurate. The only variation observed was the structural catwalks anoear to vary slightly from what is shown on the plans.
