Brandon,
Permit NCG030613 is paid up on fees now, and we did not send them a fees owed letter in 2018. However, on
10/22/2018 they still owed fees (see below). That's why they were not included the renewal batch. Perhaps they did
not owe a fee until after we'd sent the notification letters, but they were late paying by the time we prepared the
renewal flip. (We re -run those pulls to make sure we drop anyone who has paid up.)

Permit NCG050372 owed fees in 2018 as well. That letter is attached. They are also paid up now and could be renewed.
Both permittees should review their permit contact summary reports to ensure the information is up to date and then
request that Suzanne McCoy renew their COCs.
Unless a permittee that owes fees notifies us that fees have been paid up, we do not renew the COC. We would be
constantly sweeping back over to see who isn't delinquent anymore after a mass renewal, and we don't have the
resources to chase that. So, many of these remain expired after they've resolved any fees. Permittees should be making
sure they have an up-to-date COC when they review their SPPP records annually.

Subject: Permit Status
Good Morning Bethany,
I was looking at the permit status for NCG030613 (Matcor Metal — Davidson Co.) and NCG050372 (Unilin Flooring —
Davidson Co.). According to BIMS both of these are covered under expired permits. Both of these facilities are active
and during a complaint response I inspected both. They are both active and if expired truly means expired and they
aren't properly covered I was going to send out NOVs to the facilities, I just wanted to make sure I understood the
permit status.
Thanks,
Brandon L. Wise

NPDES Permit Certificate of Coverage (COC) Number: NCG050372
Dear NPDES Permittee:
The General Permit under which you are covered expires on May 31, 2018. Our records indicate that
annual fees are owed on this permit. Coverage under this permit for this site cannot be renewed until
these fees are paid.
Sometimes fees lapse because billing contact or other information is out of date. Please review the
permit contact information enclosed. Ensure that all contact information is accurate and complete, and
that e-mail addresses are provided wherever possible. If you no longer need this permit, please submit
a Rescission Request Form. That form is available from our website:
https://deg.nc.gov/about/divisions/energy-mineral-land-resources/npdes-industrial-stormwater
To update contact information for your permit: You may simply e-mail us changes, unless there
is a change to the Owner Affiliation. Because Owner Affiliation is the person legally responsible for the
permit, we require an Owner Affiliation Change Form to make this change. A change in company Name
or Ownership requires a different form, so please contact us if necessary, or see the website above for
links to each of these on-line forms. Changes to other contacts can be e-mailed to Laura Alexander at
laura.alexander@ncdenr.Bov.
If you have any questions, please contact Laura Alexander at the e-mail above or at (919) 807-6368, or
Deborah Reese at (919) 707-9220 or deborah.reese@ncdenr.gov.
