REGAN secretary LINDA CULPEPPER Director Jed Simpson, Mayor Town of Lucama P.O Box 127 Lucama, North Carolina 27851 Dear Mr. Simpson, NORTH CAROLINA Environmental Quality August 02, 2019 Subject: Compliance Evaluation Inspection NOD-2019-PC-0176 Town of Lucama Collection System Permit No.: WQCSDO009 Wilson County On July 18, 019, Erin Deck of this office conducted the subject inspection. The collection system is deemed permitted through 15A NCAC 02T .0403. The assistance of Mr. Chris Hare, was appreciated during the inspection. Findings during the inspection are as follows: 1. The wastewater collection system is classified as a CS-1. A review of the Division's database BIMS (Basinwide Information Management System) revealed that a Primary Operator in Charge (ORC) and a Back-up ORC have not been identified. Per15A NCAC 08G .0201 ResyonsibilitV a S stem Owners to Designate certified operators_Owners of classified water pollution control systems shall designate operators certified by the Commission of the same type and grade as the classification for the system and for each classification shall. (1) designate one ORC who possesses a valid certificate of the type and grade at least equivalent to the type and grade of the system; (2) designate one or more Back -tip ORC(s) who possesses a valid certificate of the type of the system and no more than one grade less than the grade of the system, with the exception of no Back-up ORC is required for systems whose minimum visitation requirements are twice per year. 2. A review of Records indicated the following a. An operation and maintenance plan was not available for review. Per 15A NCAC 02T.0403(3) an operation and maintenance plan including pump station inspection frequency, preventative maintenance schedule, spare parts inventory, and overflow response shall have been developed and implemented. b. There are five high priority Iine for the system. No documentation of the high priority line inspections was available at the time of the inspection. Per 15A NCAC 02T.0403 (5), the lines must be inspected at least once every six -months and inspections documented. c. Documentation of a Grease Control Program was not available. grease education materials are distributed to users was not available for review. Per 15A NCAC 02T.403(a)(8) (A)(C), grease education material is to be distributed at least bi- annually, an ordinance and documentation of grease interceptor inspections were not available. 3. There are five pump station in the collection system; none are equipped with telemetry and per Mr. Hare, are inspected daily. Documentation of these inspections was not available for review. Three pump stations were inspected during the inspection. All had signage, restricted access and are accessible in all weather conditions. a. Pump Station #1/Cabin Branch: The visual alarm was not working at the time of the inspection. b. Pump Station #3/ Teague: As noted in the September 2017 inspection conducted by DWR staff, the generator still does not work, and there are no spare generators available if there is a power failure. c. Pump Station #6/Little Rock Acres: Audible and visual alarms were not working at the time of the inspection. 4. Three of the Town's four high -priority lines were inspected; Teague, Williams Street and Anne Little's. All appeared to be rust free and the right of ways have been maintained. The high priority line next to Ms._LittIe's home is not_straight_and appears to require maintenance. Required Response: Please respond to issues listed in 1, 2(a), 2(b), 2(c), 2(d), 3(a), 3(b), 3(c) and 4 within 30 days of receipt of this letter.
