Explain the purpose of the proposed project:* The purpose of the project is to cover a sanitary sewer line that has been exposed in the left streambank due to erosion, and stabilize the streambank. 41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* The project will involve rebuilding approximately 35 linear feet of streambank by rebuilding the streambank at a 2.5:1 slope to cover the sewer line, and to stabilize the streambank with Class 1 and 2 riprap. A tree stump located in the right streambank has caused the channel flow to deflect into the eroded streambank; therefore, this stump will be removed and the right streambank will be stabilized at this location. 4j. Please upload project drawings for the proposed project 5. Jurisdictional Determinations 5a. Have the wetlands or streams been delineated on the propertyor proposed impact areas?* r Yes r No r Unknown Comments: the entire left floodplain is wetlands. 5b. Several temporary stream crossings (temporary access roads) over Walnut Creek and a tributary to Walnut Creek at different locations will be constructed as part of the Walnut Creek interceptor rehabilitation project, but these will be permitted separately. These impacts will allow equipment access to the sanitary sewer easement at several locations (remote from this current project location) to perform rehabilitation work (installing linings in the existing sewer pipes). This stabilization project will be conducted concurrently with the sewer line rehabilitation work. D. Proposed Impacts Inventory 1. Impacts Summary 1a. Avoidance and Minimization 1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: The amount of streambank stabilization is limited to the area where the sewer line is exposed. Access to the project area Will be from the north via eAsting sanitary sewer easements. Mud mats will be used for all access routes in wetlands. 1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: Work will be conducted during a period of low (base) flow. As noted above, mud mats will be used to prevent impacts to Wetland areas. The ingress/egress and the work area are within wasting sanitary sewer easements. No trees will be removed as part of this project. 2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State 2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State? r Yes r No 2b. If this project DOES NOT require Compensatory Mitigation, explain why: Streambank stabilization does not require compensatory mitigation. F. Stormwater Management and Diffuse Flow Plan (required by DWR) 1. Diffuse Flow Plan 1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules? r Yes r No What type of SCM are you providing? r Level Spreader r Vegetated Conveyance (lower SHWT) r Wetland Swale (higher SMWT) r Other SCM that removes minimum 30% nitrogen V Proposed project will not create concentrated stormwater flow through the buffer Diffuse Flow Documentation 2. Stormwater Management Plan 2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?* r Yes r No 2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)? r Yes r No Comments: Project will not result in concentrated stormwater flow. Original ground elevations/contours vdll be maintained. No impervious surface will be added beyond riprap on the stabilized streambanks. G. Supplementary Information 1.
