2j. Provide a brief description of this project (attach a site plan): The purpose of this project is to install a gravity
sewer main, consisting of 6,865-feet of 30-inch sewer, 478-feet of 16-inch sewer, 1,203-feet of 12-inch sewer,
88-feet of 10-inch sewer, and 53-feet of 8-inch sewer. The purpose of the MODIFICATION is to change the
method of Stream Crossing #1-1 from a bore to an open cut installation. Also, the impact would change from
temporary to permanent. Please see attached Site Plan Map.
2k. Provide directions from the nearest named town, including street names and/or highway numbers: The site is
located in Cary, in the western portion of Wake County, North Carolina. The installation will begin at a point
approximately 400 feet east of the intersection of Beaver Dam Road (SR1615) and Green Level West Road (SR
1605). The site of the MODIFICATION is located at 108 Panoramic Court. The actual location of the
MODIFICATION is down a gravel driveway, behind the Town of Cary's Fieldstone Pump Station. Please see the
attached Vicinity Map.
Please attach a map of the site location using USGS 1:24,000 topographic map and a copy of the latest bound
and published County soil survey with the project/site boundaries delineated. A site plan should also be included
for all projects other than stream and wetland determinations.
