Trade St. Suite 200 Winston-Salem, NC 27101 NORTH CAROLINA Environmental Quality July 3, 2019 Subject: Request for Additional Information Stormwater Permit No. SW3190504 Line 118 Retrofit Union County Dear Mr. Jester: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on May 20, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Please provide two copies of a plan sheet showing the existing conditions of the site. 2. 15A NCAC 02H(2)(b) requires all corporations or limited liability corporations to submit documentation showing that the corporation is active and in good standing with the NC Secretary of State and documentation from the NC Secretary of State or other documentation showing the titles and positions held by the person who signed the application. See the attached pages (3-5) for the business results search from the NC Secretary of State's website, which verifies that Piedmont Natural Gas, Inc. is active and in good standing. However, the individual who signed the application (Oluwasanu Odugbesan) is not listed as a company official. Official documentation or a signed letter from a company official indicating that this individual has the authority to apply on behalf of the company is required to issue the permit. 3. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https:Hedocs.deq.nc.gov/Forms/SW Project_ Submittal Please note that this request for additional information is in response to a preliminary review. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
