Dear Mr. McIntosh:
On April 25, 2019, Paul DiMatteo of the Division of Water Resources (Division),Winston-Salem Regional Office
met with Aarnold Allred, Operator in Responsible Charge (ORC), and Mark Grose, backup-ORC, both of SUEZ,to conduct
a Compliance Evaluation Inspection of the above referenced Wastewater Collection System.
This inspection consists of(1) a review of the permit and accompanying documentation to determine
compliance with permit conditions, and (2) an on-site inspection of several collection system components. The
attached inspection form notes the areas that were evaluated for the inspection, with notable findings
outlined as follows.
Site Inspections
1. Pump Stations—The West Ridge, Foushee,and NC-22 pump stations were inspected.The visual alarm at the
NC-22 pump station did not illuminate when tested. No other issues were noted.
2. Manholes—A manhole immediately upstream of the wet well at West Ridge pump station was inspected and no
issues were noted.
Permit Conditions
1. Capital improvement plan—the operators produced a detailed analysis of the collection system produced in
2010 that included a description of the collection system,a detailed map, known deficiencies, cost analysis,and
an initial plan describing planned upgrades. However,the plan should be periodically updated to include funding
for reinvesting into the collection system for general and specific projects.
2. Annual compliance report—the operators could not produce a recent annual compliance report and the
inspector did not find such a report on the Town's website.The operators said that the Town maintained
responsibility for completing the annual report.
3. Public Notice for Sanitary Sewer Overflows—proof of publication should be kept with the relevant sewer
overflow records.
4. Gravity sewer cleaning requirement—operator records showed that they did not complete the 10%annual
cleaning requirement for the years 2017 and 2018. In 2017,the operators explained that they were not able to
complete the cleaning due to SUEZ having taken over the collection system during the year. In 2018,they
completed 11,092 feet of cleaning based on 21 miles of gravity sewer. However,the current Permit and the

most recent permit application both indicate 34 miles of gravity sewer,which means that about 17,950 feet of
sewer is required to be cleaned each year. If you believe the permit to be wrong, please submit your justification
via the appropriate channels.
Required Action
This office recognizes that with the recent shift of operational duties to another entity there may be some duties
for which there is no clear responsible party.These duties should be reviewed and the responsible party should be
identified.Additionally, all documentation showing compliance with the Permit's conditions should be available for
review during the inspection, no matter which entity is responsible for that particular condition.
If available, please provide copies of the current capital improvement plan, annual compliance reports for the
past 3 years, and justification for the reduction in gravity sewer system length as a response to this letter within 30 days
of receipt.

Inspection Summary:
According to the ORCs, SUEZ's contract with the Town includes daily inspections for all pump stations, not just those that
have only AV alarms.
Follow up/non-compliant items:
-Capital improvement plan-should have a budgetary plan that extends 3-5 years into the future.
-Annual compliance report has not been produced
-Proof of publication should be kept for SSOs that require public notice.
-10% annual gravity sewer system cleaning has not been completed. Per ORCs, in 2017 they did not complete 10%
because SUEZ took over the collection system halfway through the year, and in 2018 they completed 11,092 feet of
cleaning based on 21 miles of gravity sewer. However, the Permit states 34 miles of gravity sewer(-17,950 feet of cleaning
required per year). If the Permits numbers are inaccurate, it should be corrected.
