Hi Joe,
I'm reviewing the NOI application for Fayetteville Annexation Phase V, and everything looks good except an old version
of the NOI Certification Form was used and the boxes weren't checked on it. Attached is the new version of the NOI Cert
Form. Please fill it out, check the boxes, and email a scanned copy to me. If I receive the form by close of business today I
can approve the NOI application, otherwise I will have to reject it because the system only lets us keep applications open
for so long.
