The cover letter states that a Certificate of Public Convenience and Necessity was included, but there wasn't one in the package. However, Section VIA was checked "N/A", and it doesn't appear that such a certificate is necessary. Please confirm this. No Cert. is needed or included with this application. 3. The cover letter also states that an Operational Agreement was included, but there wasn't one in the package. However, Section V1.2 and V1.3 were checked "N/A", so it doesn't appear that an Agreement is necessary. Please confirm this. No Operational agreement is needed or included with this application. 4.
