Upon review of your request for 401 Water Quality
Certification to cross Buffalo Creek for aerial sewer line
crossing located at Hwy 6 & I-85 in Guilford County as described
in your submittal dated 8 July 1993, we have determined that the
proposed fill can be covered by General Water Quality
Certification No. 2664 issued January 21, 1992. A copy of the
General Certification is attached. This Certification may be
used in qualifying for coverage under Corps of Engineers' General
Permit No. 049.
If this Certification is unacceptable to you, you have the
right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this Certification. This
request must be in the form of a written petition conforming to
Chapter 150B of the North Carolina General Statutes and filed
with the Office of Administrative Hearings, P.O. Box 27447,
Raleigh, N.C. 27611-7447. Unless such demands are made, this
Certification shall be final and binding.

We appreciated your review of the subject project. The purpose of this letter is to transmit the completed
application for the subject project for coverage under Nationwide Permit #12 for the Aerial Creek Crossing.
Please find attached one (1) 8-1/2" x 11-0" plan and profile of the gravity ity sewer extension illustrating the
aerial crossing of Buffalo Creek and the completed Nationwid mit application. Also, by copy of this
letter, we are forwarding Mr. John Dorney of NCDEHNR-DEM (7) seven copies of the application and the 8-
1/2" x 11-0" plans for issuance of the required 401 certification.
