We don't have any other forms on file here showing otherwise. We allow permittees to send in electronic copies so the
one you sent yesterday appeared to a new submission (backdated). If that is one of your old forms, we did not
receive/process it. Forms need to be emailed to: [e-mail], or faxed to: 919-715-2726. It is the
permittee/system owner's responsibility to confirm that their form was processed and operators designated — if that
task is delegated to an ORC, that is okay, but it is still the responsibility of the owner and should be confirmed. We have
no way of know who, how or whether the form you sent me yesterday was received here in 2014. Our system and our
permit files do not have it on record.
The permittee's signature needs to be dated on every form so we know it's current, every time a change is made and
your DEQ Mooresville Regional Office should be notified every time
When I looked at the form you sent yesterday with no date for Paul Wilson - and the designations were from 2014 but
not showing in our system/files ... we just can't assume it was sent and/or received in 2014.
Designation forms must be entered on our system to exist. EDMR is a different system. The permittee has to give
permissions to the operators and I don't know what that process entails. EDMR permissions is managed by that
program.
I recommend a current form be filled out, signed, dated. We'll process it once we receive it. Quickest method is to
email it to: [e-mail].

Thanks Maureen. I am trying to remember back when these forms were first submitted with Jeff as Back-up ORC and
when ownership first changed from Amaral to Smith. I apologize, it's been a while.
1. Your ORC status predates the form you provided so, if nothing has changed, you don't need to provide your
info/signature/effective date. ---Thanks, I'll keep this in mind in the future.
2. Designations are not back dated. Please submit a new/current designation form if you wish to designate Mr.
Campbell. We can fill out additional forms but for the record none of these forms have been back dated. These are my
copies of what was submitted at that dated time period. I'm not sure what happened but Jeff Campbell has been
reviewing EDMRs as the Back-up ORC for years just as the forms indicate... Someone with the state had to allow his
permissions in the system.
3. The permittee needs to sign and date each change in designations as it's their responsibility to do so (per 15A NCAC
08G .0201). I'm confused — every one of the attached forms have Paul Wilson's signature.
