Upon review of your request for 401 Water Quality Certification to place fill material in
less than one acre of wetlands or waters which are tributary to Muddy Creek for sewer line
instation located 800 feet South of NC 67 in Forsyth County, as described in your submittal
dated 7 May 1993, we have determined that the proposed fill can be covered by General
Water Quality Certification No. 2664 issued January 21, 1992. A copy of the General
Certification is attached. This Certification may be used in qualifying for coverage under
Corps of Engineers' Nationwide Permit No. 12.

15. I contacted the Raleigh office of the U.S. Fish and Wildlife Service by phone. They
stated that unless there were 30 year or older pine trees in the permit area, there
were no endangered or threatened species. There are no 30 year or older pine trees
in this area.
16. I contacted the State Historic Preservation Office by phone and spoke with Sandra
Ward. There are no historic properties in the permit area.

Enclosed is a completed application for a Section 401, Water Quality Certification. This
project consists of an 8" V.C. sewer main crossing Muddy Creek to tie to an existing
outfall. The area beside the creek is a meadow used for hay. The tentative schedule for
construction is late summer.
