The information from the first half of 2015 cannot be found. The person who was our environmental manager passed away in April of 2015, and either the test did not get done or he filed it somewhere we can't find it. The rest of the data is based on the reporting results done by the firm we use for testing. Let me know if you need any other data not provided. Thank you for your help in this matter. I have attached an example of a spreadsheet another facility provided. I have had facilities in the past that have preferred to send copies of the monitoring data if they could not create a spreadsheet, so that is also an option. Operational changes would include examples like changes in waste production, changes in waste storage, using new/different chemicals, storing new chemicals outside and not under a cover, vehicle maintenance on the site, etc. If the facility is still producing the same products, using the same materials, and following the same procedures as in 2009, there would be nothing to be reported. First, what data needs to be on the spreadsheet? We have the monitoring data needed, I just wasn't sure what the columns in the spreadsheet need to contain. We will be glad to compile it into a spreadsheet. Second, what type of operational changes do you need to know about? We will be glad to give you any information on such changes. I have looked through our paper file and checked our database. The NCS000061 permit is categorized as "In Review," which means that we do not have it officially recorded that a finalized permit has been issued to you. The most recent permit we have on file is from 2009. Isaiah Reed has also confirmed that he did not issue a permit. I think you must be looking at an inspection report. I can see in our database that an inspection took place on April 22, 2018. I am working on issuing you a renewed permit, as the facility's permit expired in 2014. Jack DeBeve submitted a renewal application in 2014. For reference, I have attached the stormwater permit the facility should be operating under. There is some information I am required to ask you for to (1.) confirm that the information I have is correct and (2.) make sure the permit adequality serves the needs of the facility. What I need from you: A spreadsheet summarizing all of the monitoring data that has been collected since the effective date of the current permit; A verification that the information in the renewal application is still is still complete and correct; and An explanation of any operational changes since the renewal application was submitted.
