N \___, ROCKING HAM RECEIVED/NCDEQ/DWR COUNTY SCHOOLS JUL 15 2019 Water Quality Permitting Section July 10, 2019 TO WHOM IT MAY CONCERN: We received a "Notice of Violation and Assessment of Civil Penalty" for Bethany Wastewater Plant (NC0037001). We had previously requested remission of civil penalty from the violation letter we received dated May 3, 2019 (February samples).We mailed a remission pack back to the address listed at the end of the letter.This package has not been received back to our facility. When our second sample came back high, we changed the bulbs in the UV system before requesting assistance from NCDEQ. We also purchased an additional two sets of UV bulbs (see attached invoice). On March 13, 2019, we met with Gary Hudson to discuss higher than normal fecal coliform results as shown on our March 2019 eDMR report. Per Mr. Hudson's recommendation,we disassembled the UV system and cleaned the protective sleeves. We have implemented a monthly cleaning schedule for the protective sleeves at the plant. After investigation of"What Changed" in our process.We discovered a possible cause. In September 2018, we changed cleaning/sanitation products to a product called Envirox(see attached SDS). I also attached a summary of Pace Analytical Fecal Coliform results. We stopped using Envirox on May 14, 2019 at that site. We are not yet sure that there is a connection. Our sample that came back on June 12, 2019 fecal coliforms was none detected. We have since moved into summer limits because of flow and we will be unable to determine if that product caused our issue.
