Box 230 High Point, NC 27261 Subject: COMPLIANCE AUDIT — DOCUMENTATION REQUEST City of High Point NPDES MS4 Permit No. NCS000421 Guilford County Dear Permit Holder: The Department of Environmental Quality (DEQ) will be performing a National Pollutant Discharge Elimination System (NPDES) Municipal Separate Storm Sewer System (MS4) Permit Compliance Audit for the subject permit the week of August 19th (please let us know which days that week would be preferred). The audit is expected to take at least two or three (2/3) full days. DEQ will determine the scope of the compliance audit based on a review of provided documentation, and a draft agenda will be provided once the scope of the audit has been determined. Please submit the following permit information within seven (7) calendar days: • Current Stormwater Management Plan (SWMP) • Most recent Annual Report • Organizational Chart and a description of stormwater program responsibilities for each party • Map of the MS4 permitted area with receiving waters • Formal agreements with outside entities that implement MS4 Program components All submitted documentation will become a permanent record of the compliance audit. Additional information may be requested prior to, during and/or following the compliance audit. Please also arrange for an appropriate meeting room that will accommodate your staff, 3 staff members from DEQ, and access to any electronic tools routinely utilized to implement the MS4 Program (e.g. databases, GIS, etc.).
