SW3190502 Application Page 6.pdf
Andy and Blake,
I looked over my notes for this project and it looks like everything in the design is in compliance with
the MDC but the application was signed incorrectly. I attached Page 6 of the application, where the
Property Owner's Certification is signed. This is only for instances where the applicant is not the
property owner. The Applicant's Certification at the bottom must be signed and notarized for all
applications. This will need to be updated and I'll need an original signed hardcopy.
Also, the stormwater regulations in 15A NCAC 02H .1042 require documentation from the NC
Secretary of State or other official documentation showing the titles and position held by the person
who signed the application. This is to ensure that the person who submitted the application has the
authority to do so. I searched the Secretary of State's website for the business registration and the
person who signed, Caleb Moore, isn't listed on the website. We can accept a signed letter or other
documentation from the company stating their title or authority to apply if that works for them.
Let me know if you have any questions.
