I reviewed your NOI submission for Mason Spring Acres, and there are some corrections that need to be made
before approval can be granted.
Part Al —project name should match what is on Letter of Approval since the approval is for lots 10 and 11, not
just 11.
Part Al2— acres to be disturbed must match what is on Letter of Approval.
Part A15a,b — according to the Surface Water Classifications Map, your sites drain to Lippard Creek. Please
verify.
Part D6 —the NOI Certification Form must be downloaded, filled out, and then uploaded.
Part E—your signature must include both your first name and last name.
Attached to this email are tips for filling out the NOI and a copy of your previous submission.
