GENERAL CONSTRUCTION NOTES
1. UNLESS OTHERWISE STATED, ALL FILL AREAS SHALL BE CONSTRUCTED IN LAYERS OF 8" MAXIMUM THICKNESS,
WITH WATER ADDED OR SOIL CONDITIONED TO THE OPTIMUM MOISTURE CONTENT AS DETERMINED BY THE
ENGINEER AND COMPACTED WITH A SHEEP'S FOOT ROLLER TO A COMPACTION EQUAL TO OR GREATER THAN
95% (100% IN THE TOP 2' OF THE SUB GRADE BELOW ROADWAYS AND PARKING LOTS) OF THE DENSITY
OBTAINED BY COMPACTING A SAMPLE OF THE MATERIAL IN ACCORDANCE WITH THE STANDARD PROCTOR
METHOD OF MOISTURE -DENSITY RELATIONSHIP TEST, ASTM D698 OR AASHTO-99 UNLESS SPECIFIED IN OTHER
SPECIFICATIONS.
2. ENTIRE AREA TO BE GRADED SHALL BE CLEARED AND GRUBBED. NO FILL SHALL BE PLACED ON ANY AREA
NOT CLEARED AND GRUBBED.
3. ALL SOIL EROSION CONTROL MEASURES REQUIRED BY THE GRADING PLAN SHALL BE PERFORMED PRIOR TO
GRADING, CLEARING OR GRUBBING. ALL EROSION CONTROL DEVICES SUCH AS SILT FENCES, ETC., SHALL BE
MAINTAINED IN WORKABLE CONDITION FOR THE LIFE OF THE PROJECT AND SHALL BE REMOVED AT THE
COMPLETION OF THE PROJECT ONLY ON THE ENGINEER'S APPROVAL. PAYMENT SHALL BE CONSIDERED
INCIDENTAL TO CLEARING AND GRUBBING. IF DURING THE LIFE OF THE PROJECT, A STORM CAUSES SOIL
EROSION WHICH CHANGES FINISH GRADES OR CREATES "GULLIES" AND "WASHED AREAS", THESE SHALL BE
REPAIRED AT NO EXTRA COST, AND ALL SILT WASHED OFF OF THE PROJECT SITE ONTO ADJACENT PROPERTY
SHALL BE REMOVED AS DIRECTED BY THE ENGINEER AT NO EXTRA COST. THE CONTRACTOR SHALL ADHERE
TO ANY APPROVED EROSION CONTROL PLANS WHETHER INDICATED IN THE CONSTRUCTION PLANS OR UNDER
SEPARATE COVER.
4. DISPOSABLE MATERIAL
A. CLEARING AND GRUBBING WASTES SHALL BE REMOVED FROM THE SITE AND PROPERLY DISPOSED OF BY
THE CONTRACTOR AT HIS EXPENSE, UNLESS SPECIFIED OTHERWISE. TOPSOIL MAY BE PLACED UP TO 6"
IN DEPTH ON SLOPES AND OPEN AREAS IF AVAILABLE WITH PRIOR APPROVAL BY ENGINEER.
B. SOLID WASTES TO BE REMOVED SUCH AS SIDEWALKS CURBS PAVEMENT ETC., MAY BE PLACED IN
SPECIFIC DISPOSAL AREAS DELINEATED ON THE PLANS OR REMOVED FROM THE SITE AS REQUIRED BY
THE SPECIFICATIONS. THIS MATERIAL SHALL HAVE A MINIMUM COVER OF 2'. THE CONTRACTOR SHALL
MAINTAIN SPECIFIED COMPACTION REQUIREMENTS IN THESE AREAS. WHEN DISPOSAL SITES ARE NOT
PROVIDED, THE CONTRACTOR SHALL REMOVE THIS WASTE FROM THE SITE AND PROPERLY DISPOSE OF IT
AT HIS EXPENSE.
C. ABANDONED UTILITIES SUCH AS CULVERTS, WATER PIPE, HYDRANTS, CASTINGS, PIPE APPURTENANCES,
UTILITY POLES, ETC., SHALL BE THE PROPERTY OF THE SPECIFIC UTILITY AGENCY, OR COMPANY HAVING
JURISDICTION. BEFORE THE CONTRACTOR CAN REMOVE, DESTROY, SALVAGE, REUSE, SELL OR STORE FOR
HIS OWN USE ANY ABANDONED UTILITY, HE MUST PRESENT TO THE OWNER WRITTEN PERMISSION FROM
THE UTILITY INVOLVED.
D. ON SITE BURNING IS PROHIBITED.
5. THE CONTRACTOR IS RESPONSIBLE FOR THE COORDINATION OF ADJUSTMENT OF ALL UTILITY SURFACE
ACCESSES WHETHER HE PERFORMS THE WORK OR A UTILITY COMPANY PERFORMS THE WORK.
6. THE CONTRACTOR SHALL CONTROL ALL "DUST" BY PERIODIC WATERING AND SHALL PROVIDE ACCESS AT ALL
TIMES FOR PROPERTY OWNERS WITHIN THE PROJECT AREA AND FOR EMERGENCY VEHICLES. ALL OPEN DITCHES
AND HAZARDOUS AREAS SHALL BE CLEARLY MARKED IN ACCORDANCE WITH THE SPECIFICATIONS.
7. ALL AREAS WHERE THERE IS EXPOSED DIRT SHALL BE SEEDED, FERTILIZED AND MULCHED ACCORDING TO THE
SPECIFICATIONS. THE FINISHED SURFACE SHALL BE TO GRADE AND SMOOTH, FREE OF ALL ROCKS LARGER
THAN 3", EQUIPMENT TRACKS, DIRT CLODS, BUMPS, RIDGES AND GOUGES PRIOR TO SEEDING; THE SURFACE
SHALL BE LOOSENED TO A DEPTH OF t4"-6" TO ACCEPT SEED. THE CONTRACTOR SHALL NOT PROCEED WITH
SEEDING OPERATIONS WITHOUT FIRST OBTAINING THE ENGINEER'S APPROVAL OF THE GRADED SURFACE. ALL
SEEDING SHALL BE PERFORMED BY A MECHANICAL "HYDRO -SEEDER". HAND SEEDING SHALL BE AUTHORIZED
ON AN AREA BY AREA APPROVAL BY THE ENGINEER. DISTURBED AREAS SHOULD THEN BE MATTED WITH
SHORT-TERM PHOTODEGRADABLE EROSION CONTROL MATTING.
8. WHERE SPECIFIED, STORM DRAIN PIPE SHALL BE REINFORCED CONCRETE PIPE (RCP) CONFORMING TO AASHTO
M-170, AS CONTAINED IN NCDOT STANDARD SPECIFICATION 1032-9 FOR WALL TYPE.
WHERE SPECIFIED, ALL STORM DRAIN PIPE SHALL BE HIGH DENSITY POLYETHYLENE (HDPE). SMOOTH WALL
INTERIOR, WITH WATER TIGHT JOINTS, BACKFILLED WITH # 57 WASHED STONE UP TO MIN. 6" OVER THE TOP
OF THE PIPE. HDPE PIPE USED FOR STORM DRAINAGE DETENTION SYSTEMS SHALL BE ADS N12 WT
OR APPROVED EQUAL.
9. CONTRACTOR SHALL VERIFY ALL ELEVATIONS BEFORE INSTALLATION OF FACILITIES.
10. CATCH BASINS CAST -IN-PLACE SHALL CONFORM TO THE REQUIREMENTS OF NCDOT STANDARD SPECIFICATIONS
FOR ROADS AND STRUCTURES (LATEST EDITION) ARTICLES 840-1 THROUGH 840-3. CURB INLET CATCH BASIN
SHALL CONFORM TO NCDOT STANDARD DETAILS 840.02 THROUGH 840.04. DROP INLETS SHALL CONFORM TO
STANDARD DETAIL 840.14. JUNCTION BOXES SHALL CONFORM TO STANDARD DETAIL 840.31.
11. CURB INLET FRAME, GRATE AND HOOD SHALL BE NEENAH R -3233D, PRODUCTS BY DEWEY BROS., U.S.
FOUNDRY OR EQUAL. DROP INLET FRAME AND GRATE SHALL BE NEENAH R -3339A OR EQUAL. FIELD INLET
COVER SHALL CONFORM TO NCDOT STANDARD DETAIL 840.04, OPENING FACING UPSTREAM.
12. CONCRETE AND MASONRY SHALL MEET THE REQUIREMENTS OF APPROPRIATE SECTION OF NCDOT STANDARD
SPECIFICATIONS FOR ROAD AND STRUCTURES (LATEST EDITION). CONCRETE SHALL BE CLASS A OR B, 4000
PSI MINIMUM, MEETING THE REQUIREMENTS OF SECTION 1000, CONSTRUCTED IN ACCORDANCE WITH SECTION
825. MASONRY SHALL MEET THE REQUIREMENTS OF SECTION 1040, CONSTRUCTED IN ACCORDANCE WITH
SECTION 830 AND/OR 834.
13. TOPS OF PROPOSED FRAMES AND GRATES SHALL BE FLUSH WITH FINISHED GRADE.
14 NO BASE COURSE SHALL BE PLACED ON MUCK, PIPE, CLAY, ORGANIC MATTER, OR OTHER UNSUITABLE
MATTER, AND A MINIMUM COMPACTION RATE OF SUBGRADE PRIOR TO PAVING SHALL NOT BE LESS THAN 95%
BY STANDARD PROCTOR METHOD AND CERTIFIED BY A PROFESSIONAL ENGINEER.

1. PROJECT SITE CONTAINS JURISDICTIOANL WATERS OF THE U.S (WOTUS).
2. PROJECT INVOLVES IMPACTS TO WOTUS IN SEVERAL LOCATIONS.
3. PROPOSED IMPACTS TO WOTUS ARE REGULATED BY THE CLEAN WATER ACT
SECTIONS 404 AND 401.
4. PERMITS HAVE BEEN OBTAINED BY THE CITY OF XXX INC TO AUTHORIZE THE
PROPOSED IMPACTS TO WOTUS AND ARE SUBJECT TO THE TERMS AND
CONDITIONS OF THE PERMITS.
5. CWA 404/401 PERMIT REQUIREMENTS, TERMS, AND CONDITIONS ARE SUMMARIZED
ON THIS SHEET ALONG WITH VARIOUS ASSOCIATED DETAILS AND SPECIFICATIONS.
6. A COPY OF THE PERMITS HAVE BEEN PROVIDED TO THE CONTRACTOR AT THE

B) COMMUNICATION OF PERMIT COMPLIANCE RESPONSIBILITIES TO SUBCONTRACTORS
WORKING ON THE PROJECT.
C) COORDINATION WITH PROJECT ENGINEER REGARDING PERMIT COMPLIANCE RELATED
ISSUES AND QUESTIONS.
D) IMMEDIATE REPORTING TO PROJECT ENGINEER OR OWNER OF ANY VIOLATION OR
SUSPECTED VIOLATION OF PERMIT CONDITIONS.

NATIONWIDE PERMIT CONDITIONS
1. AQUATIC LIFE MOVEMENTS - NO ACTIVITY MAY SUBSTANTIALLY DISRUPT THE NECESSARY LIFE
CYCLE MOVEMENTS OF THOSE SPECIES OF AQUATIC LIFE INDIGENOUS TO THE WATERBODY,
INCLUDING THOSE SPECIES THAT NORMALLY MIGRATE THROUGH THE AREA UNLESS THE ACTIVITY'S
PRIMARY PURPOSE IS TO IMPOUND WATER. ALL PERMANENT AND TEMPORARY CROSSINGS OF
WATERBODIES SHALL BE SUITABLY CULVERTED, BRIDGED, OR OTHERWISE DESIGNED AND
CONSTRUCTED TO MAINTAIN LOW FLOWS TO SUSTAIN THE MOVEMENT OF THOSE AQUATIC SPECIES.
2. SUITABLE MATERIAL - NO ACTIVITY MAY USE UNSUITABLE MATERIAL (E.G., TRASH, DEBRIS, CAR
BODIES, ASPHALT, ETC.). MATERIAL USED FOR CONSTRUCTION OR DISCHARGED MUST BE FREE
FROM TOXIC POLLUTANTS IN TOXIC AMOUNTS (SEE SECTION 307 OF THE CLEAN WATER ACT).
3. MANAGEMENT OF WATER FLOWS - TO THE MAXIMUM EXTENT PRACTICABLE, THE
PRE -CONSTRUCTION COURSE, CONDITION, CAPACITY, AND LOCATION OF OPEN WATERS MUST BE
MAINTAINED FOR EACH ACTIVITY, INCLUDING STREAM CHANNELIZATION AND STORM WATER
MANAGEMENT ACTIVITIES, EXCEPT AS PROVIDED BELOW. THE ACTIVITY MUST BE CONSTRUCTED TO
WITHSTAND EXPECTED HIGH FLOWS. THE ACTIVITY MUST NOT RESTRICT OR IMPEDE THE PASSAGE
OF NORMAL OR HIGH FLOWS, UNLESS THE PRIMARY PURPOSE OF THE ACTIVITY IS TO IMPOUND
WATER OR MANAGE HIGH FLOWS. THE ACTIVITY MAY ALTER THE PRE -CONSTRUCTION COURSE,
CONDITION, CAPACITY, AND LOCATION OF OPEN WATERS IF IT BENEFITS THE AQUATIC
ENVIRONMENT (E.G., STREAM RESTORATION OR RELOCATION ACTIVITIES).
4. EQUIPMENT - HEAVY EQUIPMENT WORKING IN WETLANDS OR MUDFLATS MUST BE PLACED ON
MATS, OR OTHER MEASURES MUST BE TAKEN TO MINIMIZE SOIL DISTURBANCE.
5. SOIL EROSION AND SEDIMENT CONTROLS - APPROPRIATE SOIL EROSION AND SEDIMENT CONTROLS
MUST BE USED AND MAINTAINED IN EFFECTIVE OPERATING CONDITION DURING CONSTRUCTION, AND
ALL EXPOSED SOIL AND OTHER FILLS, AS WELL AS ANY WORK BELOW THE ORDINARY HIGH WATER
MARK OR HIGH TIDE LINE, MUST BE PERMANENTLY STABILIZED AT THE EARLIEST PRACTICABLE
DATE. PERMITTEES ARE ENCOURAGED TO PERFORM WORK WITHIN WATERS OF THE UNITED STATES
DURING PERIODS OF LOW -FLOW OR NO -FLOW.
6. REMOVAL OF TEMPORARY FILLS - TEMPORARY FILLS MUST BE REMOVED IN THEIR ENTIRETY AND
THE AFFECTED AREAS RETURNED TO PRE -CONSTRUCTION ELEVATIONS. THE AFFECTED AREAS MUST
BE REVEGETATED, AS APPROPRIATE.
7. PROPER MAINTENANCE - ANY AUTHORIZED STRUCTURE OR FILL SHALL BE PROPERLY MAINTAINED,
INCLUDING MAINTENANCE TO ENSURE PUBLIC SAFETY AND COMPLIANCE WITH APPLICABLE NWP
GENERAL CONDITIONS, AS WELL AS ANY ACTIVITY -SPECIFIC CONDITIONS ADDED BY THE DISTRICT
ENGINEER TO AN NWP AUTHORIZATION.
8. DISCOVERY OF PREVIOUSLY UNKNOWN REMAINS AND ARTIFACTS - IF YOU DISCOVER ANY
PREVIOUSLY UNKNOWN HISTORIC, CULTURAL OR ARCHEOLOGICAL REMAINS AND ARTIFACTS WHILE
ACCOMPLISHING THE ACTIVITY AUTHORIZED BY THIS PERMIT, YOU MUST IMMEDIATELY NOTIFY THE
DISTRICT ENGINEER OF WHAT YOU HAVE FOUND, AND TO THE MAXIMUM EXTENT PRACTICABLE,
AVOID CONSTRUCTION ACTIVITIES THAT MAY AFFECT THE REMAINS AND ARTIFACTS UNTIL THE
REQUIRED COORDINATION HAS BEEN COMPLETED. THE DISTRICT ENGINEER WILL INITIATE THE
FEDERAL, TRIBAL AND STATE COORDINATION REQUIRED TO DETERMINE IF THE ITEMS OR REMAINS
WARRANT A RECOVERY EFFORT OR IF THE SITE IS ELIGIBLE FOR LISTING IN THE NATIONAL
REGISTER OF HISTORIC PLACES.
9. RESTRICTION ON USE OF LIVE CONCRETE - FOR ALL NWPS WHICH ALLOW THE USE OF CONCRETE
AS A BUILDING MATERIAL, LIVE OR FRESH CONCRETE, INCLUDING BAGS OF UNCURED CONCRETE,
MAY NOT COME INTO CONTACT WITH THE WATER IN OR ENTERING INTO WATERS OF THE US. WATER
INSIDE COFFER DAMS OR CASINGS THAT HAS BEEN IN CONTACT WITH WET CONCRETE SHALL ONLY
BE RETURNED TO WATERS OF THE US WHEN IT IS NO LONGER POSES A THREAT TO AQUATIC
ORGANISMS.
10. REQUIREMENTS FOR USING RIPRAP FOR BANK STABILIZATION - FOR ALL NWPS THAT ALLOW FOR
THE USE OF RIPRAP MATERIAL FOR BANK STABILIZATION, THE FOLLOWING MEASURES SHALL BE
APPLIED:
10.1. FILTER CLOTH MUST BE PLACED UNDERNEATH THE RIPRAP AS AN ADDITIONAL REQUIREMENT
OF ITS USE IN NORTH CAROLINA WATERS.
10.2. THE PLACEMENT OF RIPRAP SHALL BE LIMITED TO THE AREAS DEPICTED ON SUBMITTED WORK
PLAN DRAWINGS.
10.3. THE RIPRAP MATERIAL SHALL BE CLEAN AND FREE FROM LOOSE DIRT OR ANY POLLUTANT
EXCEPT IN TRACE QUANTITIES THAT WOULD NOT HAVE AN ADVERSE ENVIRONMENTAL EFFECT.
10.4. IT SHALL BE OF A SIZE SUFFICIENT TO PREVENT ITS MOVEMENT FROM THE AUTHORIZED
ALIGNMENT BY NATURAL FORCES UNDER NORMAL CONDITIONS.
10.5. THE RIPRAP MATERIAL SHALL CONSIST OF CLEAN ROCK OR MASONRY MATERIAL SUCH AS,
BUT NOT LIMITED TO, GRANITE, MARL, OR BROKEN CONCRETE.
11. SAFE PASSAGE REQUIREMENTS FOR CULVERT PLACEMENT - FOR ALL NWPS THAT INVOLVE THE
CONSTRUCTION/INSTALLATION OF CULVERTS, MEASURES WILL BE INCLUDED IN THE
CONSTRUCTION/INSTALLATION THAT WILL PROMOTE THE SAFE PASSAGE OF FISH AND OTHER
AQUATIC ORGANISMS. THE DIMENSION, PATTERN, AND PROFILE OF THE STREAM ABOVE AND BELOW
A PIPE OR CULVERT SHOULD NOT BE MODIFIED BY WIDENING THE STREAM CHANNEL OR BY
REDUCING THE DEPTH OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION ACTIVITY. THE
WIDTH, HEIGHT, AND GRADIENT OF A PROPOSED CULVERT SHOULD BE SUCH AS TO PASS THE
AVERAGE HISTORICAL LOW FLOW AND SPRING FLOW WITHOUT ADVERSELY ALTERING FLOW VELOCITY.
SPRING FLOW SHOULD BE DETERMINED FROM GAGE DATA, IF AVAILABLE. IN THE ABSENCE OF SUCH
DATA, BANKFULL FLOW CAN BE USED AS A COMPARABLE LEVEL.
EXISTING CONDITIONS LEGEND
NCDEQ GENERAL CONSTRUCTION CONDITIONS
1. THE CONTRACTOR SHALL MINIMIZE CLEARING WITHIN WETLANDS AND ACROSS STREAMS
TO THE GREATEST EXTENT PRACTICABLE AND SHALL NOT EXCEED 50' WIDE FOR GAS
UTILITY LINES AND 40' WIDE FOR ALL OTHER UTILITY LINES. EXCEPTIONS TO THIS
CONDITION WILL REQUIRE WRITTEN APPROVAL FROM THE PROJECT ENGINEER. FOR
CONSTRUCTION CORRIDORS IN WETLANDS AND ACROSS STREAM CHANNELS, STUMPS
SHALL BE GRUBBED ONLY AS NEEDED TO INSTALL THE UTILITY AND REMAINING STUMPS
SHALL BE CUT OFF AT GRADE LEVEL. THE GENERAL STRIPPING OF TOPSOIL WITHIN
WETLANDS ALONG THE CONSTRUCTION CORRIDOR IS NOT PERMITTED.
2. PERMANENT MAINTAINED ACCESS CORRIDORS IN WETLANDS AND ACROSS STREAM
CHANNELS SHALL BE RESTRICTED TO THE MINIMUM WIDTH

PIPE OR CULVERT SHALL NOT BE MODIFIED BY WIDENING THE STREAM CHANNEL OR BY
REDUCING THE DEPTH OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION
ACTIVITY.
11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION WITHIN
DISTURBED RIPARIAN AREAS SHALL COMPLY WITH ALL OTHER FEDERAL, STATE AND
LOCAL REGULATIONS. FERTILIZER APPLICATION SHALL BE ACCOMPLISHED IN A MANNER

MANAGEMENT PRACTICES FROM THE MOST CURRENT VERSION OF THE INC SEDIMENT AND
EROSION CONTROL MANUAL, OR THE NC DOT CONSTRUCTION AND MAINTENANCE
ACTIVITIES MANUAL, SUCH AS SANDBAGS, ROCK BERMS, COFFERDAMS, AND OTHER
DIVERSION STRUCTURES SHALL BE USED TO MINIMIZE EXCAVATION IN FLOWING WATER.
EXCEPTIONS TO THIS CONDITION REQUIRE WRITTEN APPROVAL FROM THE PROJECT
ENGINEER.
10. CULVERTS SHALL BE INSTALLED IN SUCH A MANNER THAT THE ORIGINAL STREAM
PROFILES ARE NOT ALTERED AND ALLOW FOR AQUATIC LIFE MOVEMENT DURING LOW
FLOWS. THE DIMENSION PATTERN AND PROFILE OF THE STREAM ABOVE AND BELOW A
PIPE OR CULVERT SHALL NOT BE MODIFIED BY WIDENING THE STREAM CHANNEL OR BY
REDUCING THE DEPTH OF THE STREAM IN CONNECTION WITH THE CONSTRUCTION
ACTIVITY.
11. APPLICATION OF FERTILIZER TO ESTABLISH PLANTED/SEEDED VEGETATION WITHIN
DISTURBED RIPARIAN AREAS SHALL COMPLY WITH ALL OTHER FEDERAL, STATE AND
LOCAL REGULATIONS. FERTILIZER APPLICATION SHALL BE ACCOMPLISHED IN A MANNER
THAT MINIMIZES THE RISK OF CONTACT BETWEEN THE FERTILIZER AND SURFACE WATERS.
12. IF CONCRETE IS USED DURING CONSTRUCTION, THEN ALL NECESSARY MEASURES SHALL
BE TAKEN TO PREVENT DIRECT CONTACT BETWEEN UNCURED OR CURING CONCRETE AND
WATERS OF THE US. WATER THAT INADVERTENTLY CONTACTS UNCURED CONCRETE
SHALL NOT BE DISCHARGED TO WATERS OF THE US.
13. ALL PROPOSED AND APPROVED TEMPORARY FILL AND CULVERTS SHALL BE REMOVED
AND THE IMPACTED AREA RETURNED TO NATURAL CONDITIONS WITHIN 60 CALENDAR
DAYS AFTER THE TEMPORARY IMPACT IS NO LONGER NECESSARY. THE IMPACTED AREAS
SHALL BE RETURNED TO ORIGINAL GRADE, INCLUDING EACH STREAM'S ORIGINAL CROSS
SECTIONAL DIMENSIONS, PLANFORM PATTERN, AND LONGITUDINAL BED PROFILE. ALL
TEMPORARY IMPACTS SHALL BE RESTORED AND STABILIZED WITH NATIVE VEGETATION.
14. ALL PROPOSED AND APPROVED TEMPORARY PIPES/CULVERTS/RIP-RAP PADS ETC. IN
STREAMS SHALL BE INSTALLED AS OUTLINED IN THE MOST RECENT EDITION OF THE NC
SEDIMENT AND EROSION CONTROL PLANNING AND DESIGN MANUAL OR THE NCDOT BEST
MANAGEMENT PRACTICES FOR CONSTRUCTION AND MAINTENANCE ACTIVITIES SO AS NOT
TO RESTRICT STREAM FLOW OR CAUSE DIS -EQUILIBRIUM DURING USE OF THIS
CERTIFICATION.
15. ANY RIP -RAP REQUIRED FOR PROPER CULVERT PLACEMENT, STREAM STABILIZATION, OR
RESTORATION OF TEMPORARILY DISTURBED AREAS SHALL BE RESTRICTED TO THE AREA
DIRECTLY IMPACTED BY THE APPROVED CONSTRUCTION ACTIVITY. ALL RIP -RAP SHALL
BE PLACED SUCH THAT THE ORIGINAL STREAM ELEVATION AND STREAM BANK
CONTOURS ARE RESTORED AND MAINTAINED, NOT RESULT IN THE DE -STABILIZATION OF
STREAM BED OR BANKS UPSTREAM OR DOWNSTREAM OF THE AREA, AND NOT RESTRICT
AQUATIC LIFE PASSAGE.
16. ANY RIP -RAP USED FOR STREAM OR SHORELINE STABILIZATION SHALL BE OF SIZE AND
DENSITY TO PREVENT MOVEMENT BY WAVE, CURRENT, OR STREAM FLOWS, AND WILL
CONSIST OF CLEAN ROCK OR MASONRY MATERIAL. RIP -RAP SHALL NOT BE INSTALLED
IN THE STREAMBED EXCEPT IN SPECIFIC AREAS REQUIRED FOR VELOCITY CONTROL AND
TO ENSURE STRUCTURAL INTEGRITY OF BANK STABILIZATION MEASURES.
17. ALL MECHANIZED EQUIPMENT OPERATED NEAR SURFACE WATERS SHALL BE INSPECTED,
FUELED, LUBRICATED, AND MAINTAINED REGULARLY AND MAINTENANCE PERFORMED IN A
MANNER TO PREVENT CONTAMINATION OF STREAM WATER FROM FUELS, LUBRICANTS,
HYDRAULIC FLUIDS, OR OTHER TOXIC MATERIALS.
18. HEAVY EQUIPMENT WORKING IN WETLANDS SHALL BE PLACED ON MATS OR OTHER
MEASURES TAKEN TO MINIMIZE SOIL DISTURBANCE.
19. THE CONTRACTOR OR OTHER RESPONSIBLE ENTITY SHALL REPORT ANY PETROLEUM
SPILL OF 25 GALLONS OR MORE; ANY SPILL REGARDLESS OF AMOUNT THAT CAUSES A

CAROLINA DEPARTMENT OF ENVIRONMENTAL QUALITY (NCDEQ) AND AUTHORIZED BY THE
CITY OF HENDERSONVILLE. THE WORK IS SUBJECT TO INSPECTIONS AT ALL TIMES BY
REPRESENTATIVES OF NCDEQ, THE CITY OF HENDERSONVILLE, THE OWNER OR THE
ENGINEER. THE PERMITS REQUIRE CERTIFICATION OF COMPLETION OF THE WATER SYSTEMS
BY THE ENGINEER AND THE APPLICANT PRIOR TO ISSUANCE OF FINAL OPERATION
APPROVAL.
2. MATERIALS AND INSTALLATION FOR WATER CONSTRUCTION SHALL CONFORM TO THE LATEST
VERSIONS OF CITY SPECIFICATION AND DETAILS AND AWWA STANDARDS AND REQUIREMENTS.
3. CONTRACTOR SHALL NOTIFY NC811 & APPROPRIATE UTILITIES AGENCIES PRIOR TO
PERFORMING ANY WORK.
4. REGULAR WORKING HOURS SHALL BE FROM 7:00 AM TO 5:00 PM MONDAY THROUGH
FRIDAY, EXCEPT IN CASES OF EMERGENCY OR OTHERWISE APPROVED IN WRITING BY THE
CITY OR AUTHORIZED REPRESENTATIVES. THE CONTRACTOR SHALL ENSURE THE
CONSTRUCTION SITE IS SAFE FOR ANY PERSONS WHO MAY BE ON SITE DURING
NON -WORKING HOURS.
5. ALL PERSONS SHALL BE COURTEOUS AND RESPECTFUL TO THE PUBLIC. CURSING OUR
FOUL LANGUAGE IS NOT PERMITTED AND WILL NOT BE TOLERATED.
6. THE CONTRACTOR IS RESPONSIBLE FOR PROVIDING TRAFFIC CONTROL ON ALL ROADWAYS
DURING THE PROJECT. THE CONTRACTOR SHALL NOTIFY LOCAL EMERGENCY, SCHOOL AND
OTHER NECESSARY AUTHORITIES PRIOR TO ANY STREET CLOSING OR TRAFFIC CHANGE.
7. THE CONTRACTOR AT HIS OWN EXPENSE SHALL KEEP THE CONSTRUCTION SITE AND
ADJACENT PUBLIC AND PRIVATE ROADWAYS CLEAN DURING THE PROJECT. THE CONTRACTOR
IS ALSO RESPONSIBLE FOR CONTROLLING DUST WITHIN THE PROJECT AREA.
8. THE CONTRACTOR IS RESPONSIBLE FOR ALL CONSTRUCTION IN ACCORDANCE WITH THE

9. CONTRACTOR SHALL VERIFY THE EXACT LOCATION AND THE ELEVATION FOR ALL UTILITIES,
DRAINAGE AND OTHER UNDERGROUND FACILITIES BOTH EXISTING AND PROPOSED, AND
SHALL NOTIFY THE ENGINEER OF ANY DISCREPANCIES OR CONFLICTS PRIOR TO
CONSTRUCTION.
10. CONTRACTOR SHALL PROTECT EXISTING UTILITIES DURING CONSTRUCTION. REPAIRS SHALL
BE MADE IN ACCORDANCE WITH APPLICABLE STANDARDS OF APPROPRIATE AGENCIES AT THE
CONTRACTOR'S EXPENSE.
11. DURING CONSTRUCTION THE CONTRACTOR SHALL MAINTAIN THE OPERATION OF EXISTING
UTILITIES WITH THE LEAST AMOUNT OF SERVICE INTERRUPTION POSSIBLE IN COORDINATION
WITH THE CITY OF HENDERSONVILLE. CONTINUOUS SERVICE, PUBLIC HEALTH AND SAFETY
CONSIDERATIONS SHALL EXCEED ALL OTHERS AND CONTRACTOR'S SCHEDULE, PLANS AND
WORK SHALL AT ALL TIMES BE SUBJECT TO ALTERATION AND REVISION IF NECESSARY FOR
THESE CONSIDERATIONS.
12. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE TEMPORARY OR PERMANENT
RELOCATION OF STRUCTURES AND UTILITIES, INCLUDING BUT NOT LIMITED TO POLES, SIGNS,
FENCES, HYDRANTS, VALVES, PIPING, CONDUITS AND DRAINS THAT INTERFERE WITH THE
POSITIONING OF THE WORK AS SHOWN ON THE DRAWINGS.
13. CONTRACTOR SHALL BE REQUIRED TO CONFORM AND COMPLY WITH ALL RESTRICTIONS
AND EASEMENT CONDITIONS AND IS RESPONSIBLE FOR ALL RELATED INCIDENTAL COSTS
INVOLVED.
14. EXISTING WATER SERVICES SHALL BE REPLACED TO THE EXISTING METER UNLESS
OTHERWISE APPROVED IN WRITING BY THE CITY. SPLICING OF THE WATER SERVICE ON THE
DOWNSTREAM SIDE OF THE METER IS NOT PERMITTED.
15. ALL DISTURBED AREAS, INCLUDING BUT NOT LIMITED TO ASPHALT, CONCRETE, DRIVEWAYS,
ROADS, LANDSCAPING, SHALL BE REPAIRED TO EQUAL OR BETTER CONDITION THAN THE
ORIGINAL SITE. GRASS AND LANDSCAPED AREAS FILL MATERIAL MUST BE REPLACED IN
TRENCH TO GRADE AS SOON AS WATERLINE CONSTRUCTION ALLOWS. ADEQUATE SEEDING
AND STRAW OR MULCH SHALL THEN BE APPLIED TO THE DISTURBED TRENCH AREA.
ADDITIONAL FILL AND SITE RESTORATION MAY BE REQUIRED WITHIN THE WARRANTY PERIOD
AT THE CITY'S DISCRETION.
16. CONTRACTOR SHALL PROVIDE EROSION CONTROL DEVICES TO CONTROL RUNOFF AS
REQUIRED. THE CONTRACTOR IS RESPONSIBLE FOR ANY FINES THAT MAY BE LEVIED DUE
TO RUNOFF FROM CONSTRUCTION.
17. IT IS THE SOLE RESPONSIBILITY OF THE CONTRACTOR TO FOLLOW ALL APPLICABLE
FEDERAL, STATE AND LOCAL HEALTH AND SAFETY REGULATIONS PERTAINING TO
CONSTRUCTION OPERATIONS.
18. ALL WATER MAINS SHALL HAVE 3 FEET MINIMUM COVER.
19. INSTALL FERROUS PIPING FOR BOTH WATER AND SEWER WITHIN 10 FT. OF A CROSSING

20. MAINTAIN 18 INCHES VERTICAL SEPARATION BETWEEN STORM DRAIN AND WATER, OR
INSTALL FERROUS MATERIAL WATER LINE WITHIN 10 FEET EACH SIDE OF CROSSING.
21. TAPS GREATER THAN ONE -INCH IN DIAMETER IN EXISTING, ACTIVE WATER MAIN ARE TO BE
PERFORMED BY THE CITY UNLESS OTHERWISE APPROVED IN WRITING BY THE CITY. TAPS IN
NEW CONSTRUCTION ARE TO BE PERFORMED BY THE CONTRACTOR IN ACCORDANCE WITH
CITY REQUIREMENTS.
22. ALL WATER METERS WILL BE SET BY CITY STAFF UNLESS THE CONTRACTOR IS GRANTED
WRITTEN PERMISSION BY THE CITY.
22.1. THE CONTRACTOR WILL PROVIDE THE CITY NOTICE OF 10 WORKING DAYS MINIMUM PRIOR
TO THE DATE OF METER INSTALLATION.
22.2. THE CONTRACTOR WILL PROVIDE A LICENSED PLUMBER DURING THE METER INSTALLATION
TO VERIFY LINE SERVICES IN ALL MULTI -METER BOXES.
23. ALL PUBLIC WATER MAIN CONSTRUCTION SHALL BE PERFORMED BY A NORTH CAROLINA
LICENSED UTILITY CONTRACTOR.
24. ALL WATER APPURTENANCES ARE TO BE MADE USING LEAD FREE MATERIALS. PIPE,
FITTINGS, VALVES, HYDRANTS, PIPE CLAMPS, RESTRAINTS, FLANGES, CASTINGS, REBAR,
HATCHES, INLETS, METER BOXES AND ALL OTHER CAST IRON COMPONENTS SHALL BE
MANUFACTURED IN THE USA.
25. RESTRAINED JOINTS BY AN APPROVED PIPE MANUFACTURER ARE TO BE USED FOR ALL
PUBLIC WATER LINES WHERE THRUST RESTRAINING IS REQUIRED IN ACCORDANCE WITH THE
PLANS AND MANUFACTURER'S SPECIFICATION. CONCRETE THRUST BLOCKS ARE PERMITTED
ONLY WHERE CONNECTIONS ARE MADE TO EXISTING WATER LINES OR WHERE THE USE OF
MECHANICAL RESTRAINT IS NOT FEASIBLE.
26. ALL FIRE HYDRANTS, VAULTS, BACKFLOW PREVENTERS, HOT BOXES AND WATER METERS
ARE TO BE INSTALLED ON RELATIVELY FLAT AREAS OUTSIDE OF PAVEMENT.
27. TAMPERING WITH OR ILLEGAL USE OF THE PUBLIC WATER SUPPLY WILL RESULT IN
SUSPENSION OF WATER SERVICE, INCLUDING DOMESTIC AND COMMERCIAL, UNTIL FINAL
APPROVAL BY THE CITY IS ISSUED. A PERSON FOUND TO BE USING WATER ILLEGALLY
SHALL BE SUBJECT TO PENALTIES OR FINES, AS PRESCRIBED AND APPROVED BY CITY
COUNCIL.
TESTING & INSPECTION:
1. NOTIFY THE CITY OF HENDERSONVILLE AND ENGINEER AT LEAST 5 WORKING DAYS BEFORE
STARTING CONSTRUCTION OF WATER FACILITIES TO SCHEDULE A PRE -CONSTRUCTION
CONFERENCE. THE ENGINEER SHALL PERIODICALLY INSPECT THE PROGRESS OF
INSTALLATION AT A MINIMUM AND SHALL COMPLETE A FINAL WATER CERTIFICATION.
2. ALL CHANGES TO THE APPROVED CONSTRUCTION PLANS AND SUBMITTALS MUST BE
APPROVED IN WRITING BY THE CITY PRIOR TO IMPLEMENTATION IN THE FIELD.
3. THE CONTRACTOR SHALL FURNISH SECURE AND PROVIDE ALL NECESSARY TESTING
MATERIALS, EQUIPMENT, LABOR. THE CONTRACTOR SHALL COORDINATE TESTING, FLUSHING,
DISINFECTION, AND BACTERIOLOGICAL SAMPLING WITH CITY INSPECTOR.
4. THE ENGINEER OR CONTRACTOR SHALL NOTIFY THE CITY CONSTRUCTION INSPECTORS 72
HOURS (MINIMUM) PRIOR TO ANY WATER MAIN OR TESTING OPERATIONS. AT A MINIMUM
CITY STAFF SHALL BE NOTIFIED PRIOR TO TAPS, TESTING, FLUSHING, DISINFECTION, AND
SAMPLING.
5. PRESSURE TEST WATER MAINS TO 200 PSI MIN. FOR A MINIMUM OF TWO HOURS IN
ACCORDANCE WITH CITY REQUIREMENTS AND AWWA C600.
6. DISINFECT WATER LINES AND PROVIDE ACCEPTABLE BACTERIOLOGICAL TEST FROM A
CERTIFIED TESTING LABORATORY IN ACCORDANCE WITH CITY REQUIREMENTS AND AWWA
C651. THE CONTRACTOR MAY ELECT TO USE THE CITY'S CERTIFIED TESTING LABORATORY.
7. PRIVATELY OWNED FIRE SYSTEMS SHALL BE APPROVED BY THE APPROPRIATE LOCAL
PERMITTING AUTHORITY. THE CITY ENGINEERING OR WATER AND SEWER DEPARTMENTS ARE
NOT RESPONSIBLE FOR ENSURING COMPLIANCE WITH APPROPRIATE FIRE PROTECTION
REGULATIONS. THE CITY DOES NOT PROVIDE ANY GUARANTEE OF ITS WATER SYSTEM'S
CAPABILITY TO DELIVER WATER FLOW AND RESIDUAL PRESSURE THE FIRE PERMITTING
AUTHORITY MAY REQUIRE.
8. ALL BACKFLOW PREVENTION ASSEMBLIES SHALL BE USC APPROVED AND REVIEWED BY
THE CITY'S. PROTECTION OF THE PUBLIC WATER DISTRIBUTION SYSTEM SHALL BE
ASSURED BY INSPECTION OF THE INSTALLATION AND TESTING OF BACKFLOW PREVENTION
DEVICES IN ACCORDANCE WITH APPROPRIATE LOCAL, STATE, AND FEDERAL REGULATIONS.
9. CONSTRUCTION WATER SOURCING MUST BE APPROVED IN WRITING BY THE CITY.
CONSTRUCTION WATER CONNECTIONS SHALL BE PAIRED WITH AN IRRIGATION METER AND
APPROVED BACKFLOW PREVENTION ASSEMBLY. THIS ASSEMBLY INSTALLATION MUST BE
TESTED BY A LICENSED PLUMBER AND TEST RESULTS SUBMITTED TO THE CITY BEFORE
USE OF CONSTRUCTION WATER.
10. SECURE FINAL WRITTEN ACCEPTANCE FROM THE CITY OF HENDERSONVILLE PRIOR TO
ACTIVATION OF THE SYSTEM. WATER SERVICE WILL NOT BE PROVIDED UNTIL WRITTEN
ACCEPTANCE OF THE SYSTEM IS ISSUED BY THE CITY.
11. THE ENGINEER SHALL SUPPLY THE CITY WITH FINAL AS -BUILT DRAWINGS AND COMPLETE
ALL PROJECT CLOSE OUT REQUIREMENTS.
THE STANDARD WATER DETAILS ARE FOR THE SOLE USE OF PROJECTS DIRECTLY FOR, OR THOSE PROJECTS IN WHICH OWNERSHIP WILL
BE TRANSFERRED TO THE CITY OF HENDERSONVILLE. THESE TYPICALS ARE INTENDED TO SHOW THE CITY OF HENDERSONVILLE'S
EXPECTATIONS FOR THE GENERAL LAYOUT, ARRANGEMENT, AND THE QUALITY OF EQUIPMENT AND MATERIALS FOR WATER DISTRIBUTION
SYSTEM ITEMS AND THEIR RELATED APPURTENANCES. IT REMAINS THE SOLE RESPONSIBILITY OF THE ENGINEER IN RESPONSIBLE CHARGE
(ERC) OF EACH APPLICATION TO DETERMINE, DESIGN TO, AND CERTIFY TO THE DESIGN PARAMETERS FOR EACH INSTALLATION. THE ERC
MUST ALSO ENSURE THE DESIGN COMPLIES WITH THE MINIMUM DESIGN CRITERIA OF NC DIVISION OF ENVIRONMENTAL QUALITY AND ALL
